AFFIDAVIT

I, ANTONIA ARCAIOS JIMENEZ, of legal age, single, Filipino

citizen and a resident of Tuburan, Cebu, after having been duly sworn to in
accordance with law, do hereby depose and say:

1. That I am a Roman Catholic having been baptized by virtue of the

Sacrament of Holy Baptism in the Archdiocesan Shrine of San
Antonio de Padua, Tuburan Cebu on August 28, 1969;

2. I was born on June 03, 1969 in Tuburan Cebu to Spouses ARTORO

YULDAN ARCAIOS and PURCENA BELLIAGONSALO
FERNANDEZ;

3. That on May 18, 2021, I went to the Archdiocesan Shrine of San

Antonio de Padua, Tuburan Cebu supposedly to get my Baptismal
Certificate but no record of my baptism was found;

4. I hereby testify and attest to the truth that I am a Baptized Roman

Catholic having been baptized at Archdiocesan Shrine of San
Antonio de Padua Tuburan on August 28, 1969 and was
solemnized by Rev. Fr. Isabelo Banzon ;

5. That I am executing this affidavit to attest to the truth of the

foregoing facts and for all other legal purposes that this may serve.
