WAIVER OF RIGHTS AND INTEREST

SABALA, Filipino and residing at Bungsuan, Dumarao, Capiz after having sworn to in
accordance with law, hereby depose and say that:
I have, in accordance with laws on Intestate Succession, individual claims, interest and
participation on the benefits granted by Overseas Workers Welfare Administration (OWWA)
in favor of my wife IMELDA GUIILLERMO SABALA, who died on 23rd of May 2021 at
time 04:15PM.
I hereby irrevocably, WAIVE, RELIQUISH, RENOUNCE, FORFEIT any claim,
rights, shares, participation or interest that I have, if any, over the BENEFITS granted by
OWWA, in favor my son MARVIN GUILLERMO SABALLA.
