major in General Education. I also successfully passed the Licensure Examination for
Teachers (LET) last December 16, 2022. During college days. I attended seminars
including orientation on field study and practice teaching, organizer of 47 th Foundation
Day Anniversary Celebration with the theme “Nature, Nurture, Natural”.

I worked as an assistant for the Master Teachers at Marcelino R, Veloso National High
School for more than ten months . I tasked to print modules, entering grades via excel,
checking outputs and projects of the learners. One of the purpose and reason in
applying Learning Support Aid is to enhance my skills and ability in interacting the
learners. It would also help me to develop personal and professional attributes as a
teacher.

I am Niño Angelo B. Manliguez, 30 years old and currently residing at Otabon, Tabango
Leyte. I graduated in Palompon Institute of Technology-Tabango Campus, Tabango
Leyte with a degree of Bachelor of Elementary Education major in General Education. I
also successfully passed the Licensure Examination for Teachers (LET) last December
16, 2022.

I went to Ormoc last two months to change one of the letter of my name then they
changed it. The data that has been written in my Notice of Admission is correct but
when I opened my leris account to apply for e-oath taking. It says “Record Mismatch”.

In line with this, I am humbly asking from your good office to change my incorrect name
in my leris account. My previous name in my leris account is Nino but my true name is
Niño.
