DEED OF ASSIGNMENT AND UNDERTAKING

TO WHOM THESE PRESENTS MAY COME:

This document has been executed relative to my outstanding loan obligation with BABY
CHIONG/AMICO, a Promissory Note, I executed in their favor, copies of which is with their files;
1. ____________________, filipino, of legal age and resident of ___________________
And employee of ____________________________HEREBY TRANSFER, CONVEY and
ASSIGN, my ATM CARD, with ___________________________Bank, in favor of and in
the possession of BABY CHIONG/AMICO, as SECURITY for the payment of the
abovementioned loan obligation;
2. Hereby giving BABY CHIONG/AMICO or its authorized representative, to withdraw
Any amount from the said ATMCARD, as payment to the abovementioned outstanding
loan account with them;
3. In the event, that I may need same ATM CARD for my personal purpose, with their
permission I may borrow the same, with the solemn promise to return the said ATM
CARD to them immediately or anytime upon their demand;
4. This Deed of Assignment/Undertaking was executed by me freely and voluntarily,
without force or intimidation and any civil, criminal or any liability whatsoever
connected to the above transaction is HEREBY WAIVED.
