Dear Dr. Leo,

I am writing this letter to explain as to why I thought that the payroll was in your possession. We

keep the payrolls in one shelf that is why when you looked for it, I know that it is complete but

when I pulled everything out, the JULY – AUGUST payroll wasn’t there so I asked ma’am Ruth

where was it and we couldn’t remember anything, but we remembered that you were asking for

payroll copies before so I said and thought that it was in your possession. It was purely an

HONEST MISTAKE. When you told me that it wasn’t in your hands, I asked everyone where is

it and ma’am Xandy told me that she took it because she needed the payrolls for OCAIS. We

really did not know that she took it. I am a person and people make mistakes just like you do

sometimes, there were times where you mistakenly accused us that we have some certain

documents wherein it is really in your possession. What I’m saying is we do make mistakes

sometimes and it doesn’t make us stupid, just makes us a person.

In the future I won’t be committing those mistakes again, I’ll make sure to photocopy and label

everything.

This letter is to inform you of my intention to resign from my position Pharmacist/BIR officer in

Divine Mercy Foundation of Urdaneta Inc. Hospital.

Thank you for the opportunity to work in this hospital. I appreciate all the guidance and kindness

you have shown me over the past 3 years.

My final day of employment will be on April 15, 2023 to make sure that I have finished every

task until the final submission of ITR, EWT and 1604. I have already talked to Mrs. Pira so she

could tell me if there are anything else that I still have to do. I have already started to organize

calendar deadlines/monthly to-do list so that the person who will replace me will not have a hard

time.
