resident of 102 Quezon Brgy. 116, Tondo, Manila City, do hereby authorize
my mother, LETICIA O. SUERTE, of legal age, residing 102 Quezon Brgy.
116, Tondo, Manila City to perform all the steps necessary for the change in
entry of my Certificate of Live Birth registered in the civil registry of
Quezon City and with an erroneous entry in the National Statistics Office.
