Appendix No.

to the RA Government's Decision

No. 1923-N of December 29, 2011

HEALTH INSURANCE

Insurance premium: 52000 AMD

Sum insured: 3800000 AMD

The package includes the following medical services in hospital conditions: acute, urgent and planned,
as well as the pre-hospital examinations, which are followed by the hospital treatment of the given
disease.

1. HOSPITAL CARE

1) diseases requiring therapeutic treatment.

2) diseases requiring surgical treatment, including neurosurgical, heart surgical and intravascular
interventions (in addition, stenting of coronary vessels is reimbursed in the amount of the cost of
a non-medicated stent), gynecological, etc.;

3) treatment of injuries (fractures, dislocations, wounds, sprains, polytraumas).

4) treatment of electric shocks, burns and frostbite.

Notes:

1. The hospital expenses, which include the costs of allocated medicines and medical supplies
(injectors, injection system, etc.), costs of necessary laboratory and instrumental examinations,
surgical and conservative treatment, hospital staff wages, are subject to reimbursement.

2. Fees set for hospital meals, special comfort rooms and personal care are not subject to
reimbursement.

3. The co-payments to be paid by the insured person in connection with diseases treated within
the framework of the state order are subject to reimbursement.

4. Those pre-hospital examinations followed by hospital treatment of the given disease are
subject to compensation.

5. The insurance is valid only in the territories of the Republic of Armenia and the Republic of
Nagorno-Karabakh.

6. Family members cannot be insured if the employee refuses to be insured.


7. Illnesses caused by force majeure (nuclear explosion, radiation, radioactive or chemical
contamination, military operations, civil war, public unrest or strikes) are not subject to
compensation.

2. DIAGNOSTIC LABORATORY AND INSTRUMENTAL RESEARCH

In acute, urgent and planned cases, laboratory and instrumental examinations carried out for
hospital treatment for the purpose of diagnosis, as well as control of the course of treatment, are
subject to reimbursement:

1) laboratory:

a. clinical,

b. biochemical,

c. immunological,

d. microbiological,

e. hormonal,

f. genetic,

g. pathohistological,

h. PCR, etc.

2) instrumental:

a. radiological,

b. computed tomography,

c. magnetic resonance imaging,

d. ultrasound examinations,

e. radioisotope studies,

f. functional examination (ECG, electromyography, spirography, doppler, duplex studies, etc.),

g. endoscopic examinations (esophagogastroduodenoscopy, colonoscopy, cystoscopy, etc.),

h. neurophysiological studies (EEG, audiometry, etc.).

3. PREVENTIVE TREATMENT

The package also includes an annual preventive medical check-up for 3000 drams (blood and
urine tests, electrocardiogram (ECG), consultation with a general practitioner).

4. EXCLUSIONS OF INSURANCE COVERAGE


The following are not considered as insurance accidents under the health insurance package and
are not subject to compensation:

1) outpatient examinations and treatment (including drug treatment);

2) medical services, medication, hospitalization performed without medical instructions;

3) treatment of diseases and conditions included in the state order program;

4) any preventive and invigorating measures, immunopreventive vaccinations, etc.

5) non-specific chronic lung diseases: emphysema, pneumosclerosis.

6) congenital anomalies and developmental defects or anatomical features,

Hereditary and genetic diseases: periodic disease, epilepsy, deformities and chromosomal
disorders, infantile cerebral palsy.

7) lesions of the intervertebral discs of the spine, degenerative dystrophic changes,


osteochondrosis.

8) diabetes and its consequences.

9) chronic diseases that require constant, conservative treatment and dynamic control, except for
their urgent (life-threatening) exacerbations during the insurance period;

10) chronic renal failure requiring hemodialysis.

11) autoimmune and systemic connective tissue diseases.

12) correction of weight and speech, surgical change of sex.

13) plastic and reconstructive surgeries performed for the purposes of improving the cosmetic,
aesthetic or psychological condition of the insured person, including those related to skin
diseases (calluses, warts, warts and moles, warts), correction of the nasal septum, their
complications, except those caused by an accident;

14) excimer laser operations of the eye.

15) dentistry.

16) non-traditional, alternative (non-scientific) methods of treatment and diagnosis, treatment


with experimental or research methods, traditional healing systems, services related to
telemedicine;

17) sanatorium-sanitary treatment and any physiotherapeutic measures.

18) restorative (rehabilitation) treatment.

19) intrauterine insemination and extracorporeal fertilization.


20) research and treatment related to reproductive (reproductive) function and erectile
dysfunction (impotence), conservative contraceptive interventions, including IUD placement and
removal, dysmenorrhea, contraceptives;

21) mainly sexually transmitted diseases (diagnosis and treatment);

22) chronic virulence, bacillosis, parasitism (hepatitis B, C, D, E, F).

23) liver failure.

24) HIV/AIDS.

25) radiation diseases.

26) prostheses of organs and limbs, endoprostheses (except as a result of an accident), medical
equipment, implants, purchase of visual and hearing devices, as well as expenses incurred for the
purchase and adjustment of other regulatory medical devices, wheelchairs, orthopedic devices,
etc., bioactive supplements, vitamin therapy, cosmetic accessories.

27) mental illnesses, their consequences and borderline states (neurosis, psychopathy, psychosis,
etc.), psychotherapist services, hypnosis.

28) expenses incurred as a result of the insured person not following the doctor's appointments or
refusing them;

29) alcoholism, drug addiction and drug addiction and diseases caused by them;

30) injuries and illnesses caused by illegal actions by the insured person.
