FORMAL OFFER OF EXHIBIT

The Petitioner, thru the Public Attorney’s Office, unto this Honorable Court, most
respectfully offers in evidence the following exhibits:

EXHIBIT NATURE and DESCRIPTION

“A” Petition dated January 8, 2019

Purpose: Exhibit “A” and its sub-markings is formally offered as part of the testimony
of the Petitioner to prove that the Petitioner has filed a Petition before the Court which
has jurisdiction to hear and decide the petition.

“B” Certificate of Indigency issued by Barangay Magsaysay,

Cadiz City

“C” Certificate of Indigency issued by CSWD of Cadiz City

“D” Affidavit of Indigency in the Client’s profile of the Public

Attorney’s Office of Cadiz City

Purpose: Exhibits “B”, “C” and “D” and their sub-markings are formally offered to prove
that Petitioner is an indigent and is entitled to the legal services of the Public Attorney’s
Office and that in accordance with the provisions of Republic Act 9406 and its
implementing rules and regulations, petitioner is exempt from paying the docket and
other legal fees incidental and subsequent to the filing of the petition.

“E” Order of the Honorable Court setting the petition for

hearing

Purpose: Exhibit “E” is formally offered to prove that the Honorable Court gave due
course to the petition and set it for hearing on March 27, 2019.
“F” Notice of Raffle

Purpose: Exhibit “F” with its sub-markings is formally offered to prove that a Notice of
Raffle which directs the Petitioner to publish the Order in Negros Weekly, a newspaper
of general circulation in the province.

“G” February 17-23, 2019 issue

“H” February 24-March 2, 2019 issue

“I” March 3-9, 2019 issue

Purpose: Exhibits “G”, “H” and “I” and their sub-markings are formally offered to prove
that the petition was published in three (3) consecutive weeks in Notice of Raffle, a
newspaper of general circulation.

“J” Affidavit of Publication

Purpose: Exhibit “J” and its sub-markings is formally offered to prove that the
publication was made by the publisher of Negros Weekly in its February 17-23, 2019
issue, February 24-March 2, 2019 issue, and March 3-9, 2019 issue.

“K” Notice of Appearance of the Solicitor General

Purpose: Exhibit “K” and its sub-markings is formally offered to prove that the Solicitor
General has entered its appearance for and in behalf of the government in the hearing
of this petition.

“L” Deputization of the Solicitor General deputizing the City

Prosecutor’s Office of Cadiz City

Purpose: Exhibit “L” and its sub-markings is formally offered to prove that the Solicitor
General has deputized the City Prosecutor of Cadiz City to appear for and in behalf of
the government in the hearing of this petition.

“M” Certificate of Live Birth with Registry No. 2003-721

Purpose: Exhibit “M” and its sub-markings is formally offered to prove the facts of birth
of Alexander Sumalali Moralista, Jr.
“N” Certificate of Live Birth with Registry No. 2007-4278

Purpose: Exhibit “N” and its sub-markings is formally to prove the facts of birth of
Alexander Sumalali Moralista, Jr..

“O” Certificate of Dedication/Baptismal Certificate

Purpose: Exhibit “O” and its sub-markings is formally offered to prove the identity and
the date of birth of Alexander Sumalali Moralista, Jr.

“P” Elementary School Permanent Record

Purpose: Exhibit “P” and its sub-markings is formally offered to prove the identity of
Alexander Sumalali Moralista, Jr.

“Q” High School Permanent Record

 Purpose: Exhibit “Q” and its sub-markings is formally offered to prove the identity of
Alexander Sumalali Moralista, Jr.

“R” Certificate of Marriage

Purpose: Exhibit “R” and its sub-markings is being offered to prove that petitioner is
married to the father of the minor Alexander Sumalali Moralista, Jr.

“S” Voter’s ID of Petitioner

Purpose: Exhibit “R” and its sub-markings is being offered to prove the identity
of the petitioner.

“T” NBI Clearance of Petitioner

Purpose: Exhibit “T” and its sub-markings is being offered to prove the identity
of the petitioner.

“U” Judicial Affidavit of Petitioner

Purpose: Exhibit “U” and its sub-markings is being offered as her direct testimony.

“V” Judicial Affidavit of Alexander Sumalali Moralista, Jr.

Purpose: Exhibit “V” and its sub-markings is being offered as the direct testimony of
the minor Alexander Sumalali Moralista, Jr.

“W” Affidavit of Acknowledgment

Purpose: Exhibit “W” and its sub-markings is being offered to prove that the father of
Alexander Sumalali Moralista, Jr. has acknowledged him.
“X” PhilHealth ID

Purpose: Exhibit “X” and its sub-markings is being offered to prove the identity of the
father of Alexander Sumalali Moralista, Jr.

“Y” Judicial Affidavit of Alexander C. Moralista, Sr.

Purpose: Exhibit “Y” and its sub-markings is being offered as the direct testimony
of Alexander C. Moralista, Sr.

PRAYER

WHEREFORE, it is most respectfully prayed of the Honorable Court that the foregoing
exhibits be admitted for purposes they are being offered.

Other just and equitable relief under the circumstances are also prayed for.
