AGREEMENT OF VOLUNTARY CUSTODY

OF MINOR CHILD

KNOW ALL MEN BY THESE PRESENTS:

This agreement was made and executed by and between:

______________________, of legal age, Filipino, single and a resident of

_____________ City, Negros Occidental, Philippines, hereinafter known as the
FIRST PARTY.

- AND –

______________________, of legal age, Filipino, married and a resident

of ___________________ City, Negros Occidental, Philippines, hereinafter
known as the SECOND PARTY.

- WITNESSETH –

WHEREAS, the First Party is the biological mother of the minor

____________ years old, who was born out of wedlock on ______________ at
___________________ City, Negros Occidental;

WHEREAS, the Second Party is the biological father of the minor child
____________________;

WHEREAS, the parties are living separately and have agreed that they shall
jointly share in the care and upbringing of the child and he shall remain under the
care and custody of the Second Party while the First Party is far away from the child
as she is working abroad. The First Party has the right to enjoy the company of the
child;

WHEREAS, each can pursue their professional endeavor free from the
interference of the other with respect to their respective income;

WHEREAS, the Second Party shall provide full support for the child
sustenance, education, clothing, shelter, utilities, medical care and recreation;

WHEREAS, by virtue of this agreement the First Party give her full consent
and authority to allow/grant the Second Party to bring their minor child
___________________ to travel anywhere in the Philippines and abroad for
vacation particularly in the United States of America, UAE, and other countries
which the Second Party desire to travel without the need of a parental consent from
the First Party;
 WHEREAS, this agreement recognizes the rights and obligations of both
parties as natural guardian of the minor child and does not in any way abdicate the
mother’ right;

WHEREAS, the parties recognizes that this agreement shall redound for the
welfare and protection of the child;

WHEREAS, nothing in this agreement is contrary to law, morals, good

customs, public order and public policy.

WHEREFORE, in view of the foregoing premises, the parties hereby

voluntarily bound themselves to the above terms.
