HOW TO BECOME A MEMBER:
1. At Home, Click “Join Now!”
2. Select the membership of your choice.
3. Select your terms.
4. Input your information.
5. Check user agreements and payments.
HOW TO ADD A NEW RESIDENT/PATIENT/CLIENT:
1. Select "My Account" from the top menu bar, "My
Careplan Library" on the left menu bar, or the
"Careplan Builder” on the left menu bar. This will take
you to your Residents Administration page.
2. Use the "Add New Resident" on the left menu bar.
3. Enter the Demographic Information of the specific
resident/patient/client. You can also add a photograph
of the said client.
4. You can also add other information from the client
through the other options such as clinical
information, history, social notes, and additional
notes.
5. After typing the information, click “Save” on the
upper left. You can choose to save and return to the
profile, save and select another resident, or save and
proceed to the plan of care.
HOW TO START A PLAN OF CARE:
1. From the Resident profile page:
2. Select "Plan of Care"
3. Here, you have the option to create your own care
plan from a blank record, add from a facility Library,
or add from the Careplans.com Master Library. All
options are located on the left menu bar under
options.
4. To add from the Master Library, select "Add from
master library". This will take you to the care plan
library.
5. Here you can select any problem. By selecting the (+)
sign next to each problem, you can see all the goals
and approaches associated with this problem in the
database.

keywords when searching for an item.
7. Select the goals and approaches you would like to
incorporate into your current care plan and select
"Import Selected Records" This will take you back to
your plan of care.
8. By selecting any section of the care plan, you can
edit all care plan information: This will include Date
Onset, Date Review, Date Resolved, 1st person,
Goals, Approaches, Categories, and Plan of Care
notes.
HOW TO ADD FROM NANDA I, NIC, AND NOC:
1. In the Options listed in the left menu bar, select Add
From NANDA-NIC/NOC
2. This will bring you to the Library of NANDA-I, NIC
and NOC Content. This library is alphabetically
organized by NANDA-I diagnoses. The NIC
Interventions and NOC Outcomes are listed under
each Diagnosis through linkages.
3. You have to select the defining characteristics and
related factors or risk factors that pertain to the
individual’s case.
4. After you’ve made your selections, you can move on
to outcomes under NOC which are organized into
suggested outcomes and additional outcomes. You’ll
be prompted to select the indicators and measurable
goal for each item.
5. After you’ve made your selections, you can move on
to select interventions under NIC which are organized
into 3 categories: priority interventions, suggested
interventions, and additional interventions.
6. Once appropriate items are selected from the library,
select “Import Selected Records” in the top left menu
bar.
HOW TO ADD DIAGNOSIS:
1. Click Diagnosis
2. Select “Add new Diagnosis”
3. Type pertinent information in the respective boxes
such as the description of the diagnosis, date onset,
date resolved, classification of diagnosis, ranking of
diagnosis, and pathophysiology.
4. To select an ICD-9 Diagnosis, just use the lookup
feature and select the appropriate diagnosis.
HOW TO FINALIZE THE CARE PLAN:
1. In the Plan of Care, Click on the text in each area to
edit the Problem, Goal, or Approach section of your
care plan. When you click on the text a new window
will open with a tab labeled, "Rational".
2. Enter your rationale and click "Save and Continue" or
"Save and Close".
3. Select the option "print the selected problem/need”
on the left menu bar.
4. This will bring up a window with all your problem
statements in it.
5. Check the boxes of the care plan statements which
you would like to print.
6. Check the box to "Include Rationales".
7. Click on "Print".
