Work Immersion Waiver

Dear Sir/Ma’am:

I, Juana Dela Cruz, grandmother of Juan Dela Cruz who is currently enrolled at
Cavite National High School Senior High School, Cavite City, under Technical Vocation
Livelihood track with the strand Information and Communications Technology on the
academic year 2022-2023, give the consent to my son to undergo immersion according
to the agreement between the school and company/institution policies, rules and
regulations such as:

1. The Work Immersion Program should further develop manipulative skills,

knowledge, values, and a stronger relationship between the school and the
industry.
2. The training is limited to practical/actual work.
3. The company is not obligated to hire the student–trainee after completion of
his/her training.
4. Compensation depends upon the discretion of the industrial management.
5. The company has the right to terminate the student–trainee at any time and
for a justifiable reason.
6. The school and company shall not be responsible/liable for any injury
sustained and sickness contracted during the period of training.
7. The duration of the Work Immersion Program shall be equivalent to 80
working hours for Technical Vocational Livelihood track with the strand
Information and Communications Technology unless otherwise agreed upon
by the Company and the School.
