I did a site inspection before commencing the job made sure the work area is safe and later i conducted a
interview with all the workers involved, mechanics, Engineers, marine crew, welders, fitters involved in the
Modification of the crane hook and block . This helped me to identify the hazards while the crew explained the
procedure and their part of participation in the job after talking to them i identified the hazards possible in their
roles and also verified the PTW and Risk assessment for the job. Vendors are also included in the PTW and equally
to be taken care of as a part of the crew including the crane specialist
I checked internal audit records for the crane and the accident incident log book to see what type of accidents or
near misses were reported during any crane operation.
I went through the synergy report among the whole fleet and pick out some control and actions for the task
required
For the additional control measure I read the company safety policy and the company rules and controls on how
the risk barriers shall be implemented for e.g. : company checklist prior starting any crane operation.
I also referred back to the observation cards and the accident record books that made me to review and decide
additional measures and actions required.

Moral, general legal and financial arguments Here in Aker offshore there is a moral responsibility of employer to provide moral confidence
towards their employees to maintain the health and safety standards in workplace and the running
work activity does not harm anyone. Lack of moral confidence may lead to accident, injury or
mental ill health of workers which also give major effects the family/friends of worker. long term
injuries have major impact on their daily lives. Mental health of co-workers also gets effected if
accident seems in front of them. Before the management standards approach introduced, it is
employer responsibility to support the workers with proper arrangements and explain health and
safety instruction softly. Aker offshore has a responsibility to ensure the safety of plant, premises
and person in workplace.
ILO C155/ILO RI64 Occupational safety and health convection, 1981 gives a clear instruction to
organisation to prevent the entire organisation from enforcement action and do all work through
legal system Aker offshore could face legal action from enforcement authorities like black listing,
prohibition notice (stopping all hazard related work), suspension notice, major fine etc. this type of
legal action definitely effects the organisation and their management system. Whenever there is a
legal action on company top management gets nervous and they do lot of work to overcome with
losses, and sometime enforcement agencies ban the organisation completely due to serious
violation.
Aker offshore may face Financial if health and safety not maintained in their organisation, due to
lack of safety organisation leads to major direct loss such as machinery loss (crane and tools used in

modification, loss in daily progress, wages of the production staff, court expenses to the lawyer,
medical cost, sick pay etc. And these cost can be calculated in the term of money.
It is generally possible to take out insurance to cover some major losses that are might foreseeable
occur in Aker offshore. For example, if any worker died or injured in workplace there is a kind of
insurance in place to pay that worker and their family compensation and to meet the employers
civil cost, it is a form of insured cost. uninsured cost cannot be claim through any insurance if any
accident took place in the organisation there will be definitely loss of; raw material, repairing cost,
clean-up cost, loss of relation between worker and employer, reputation damage in market etc.
And the impact these cost in organisation may be 8 to 36 times greater than insured cost which
leads major loss to to Aker offshore and effect hardly

Organisation is a legal requirement to provide a competent inspection team for machinery items to
controlling the risk. This should include:
