What Happened? Sub-Contractor Significant Incident
During pre-rig move operations, the rig move sub- (SI) - Fatality contractor’s driver (prior to loading a pipe basket onto Block 9 an oilfield truck), in accordance with the banksman’s st instructions, began reversing the truck. The driver 1 November 2022 followed the signal from the banksman to continue reversing, by checking both the driver’s side mirror and the passenger side mirror.
At the point the driver could no longer see the
banksman, he immediately stopped the truck, applied the hand brake, and exited the truck to check the rear area.
The driver found the banksman was caught between
the rear of the truck and the tool basket skid and unfortunately, the banksman sustained fatal injuries. What Caused This? Failure to follow and communicate task JSA and Procedure: • The banksman was standing in the line of fire (between the truck and the basket). • Truck driver failed to follow the policy of parking 1.5m away from the load. • Tire chocks were not placed as per reversing policy, to park and review safe distance, consisting of load or object to be 1.5 m away from the rear of the truck. • Banksman’s failure to communicate to the truck driver to STOP when coming close to the load. Position of Banksman at the time of incident • Banksman and the driver were working alone without additional supervision.
What Can I Do?
