This is to certify that I, DOLORES TIOXON , wife and heirs of ARMANDO
TIOXON (deceased) a bona fide resident of Barangay PANUTO, Pandan, Catanduanes of
legal age; owner of a parcel of land located at Barangay Panuto of the Municipality of
Pandan, Catanduanes with Lot No. 4903 with total size of 0.8390 ha. Here by, express my
approval and consent for the Cash for Work for Establishment of Cassava Plantation in
relation to the implementation of the KALAHI-CIDSS.NCDDP AF implementation in my
above mentioned property provided that it will only use 8390 sq.meters of said land for the
duration of 3 years to be managed by the Barangay Council of Barangay Panuto and
Operation and Maintenance Group.
