AGREEMENT
Mandatory Pre-employment Requirements Submission

TO WHOM IT MAY CONCERN,

I, ___________________________________ (Name), hereby enter into agreement to comply with the


submission of the mandatory pre-employment requirements, enumerated below. I understand and agree
that these requirements shall be submitted prior to my start date.

I understand that I shall exert effort and commitment to comply with its submission; and that in the event
some external factors may prevent and/or deter me from the completion and/or compliance of these
requirements in a timely manner, I shall provide valid evidence, where such documentation/proof may and
can result to the consideration and/or extension in the submission of the requirements.

The mandatory requirements covered by this agreement include the following:

PRIMARY REQUIREMENTS
Copy, Vaccination card Copy should clearly show date of vaccination shots
If Valid NBI Clearance is not readily available: NBI Appointment Schedule and
Original copy, NBI Clearance Receipt photocopy with Original Police Clearance
If SSS ID is not available, any of the following: E1 Form, Number Slip, Verification
Copy, SSS ID Slip, or Employee Static Information
If Philhealth ID is not available: Copy of stamped received Members Data Record
Copy, PhilHealth ID (MDR)
If PagIbig ID is not available, any of the following: HDMF Transaction Card, Loyalty
Copy, PagIbig ID Card, or stamped received Members Data Form (MDF)
If TIN ID is not available, any of the following: BIR 2316, stamped BIR 1904 or TIN
Copy, TIN ID Verification Slip
Copy of latest BIR 2316 For new hires with employer within the current year
Copy, school records
For fresh graduates whose TOR and Diploma are not available, Certificate of
(Transcript of Records and Graduation and Certified Copy of Grades
Diploma)
Mandatory government IDs (SSS, PagIbig, PhilHealth and TIN) are pre-requisite to
BPI Payroll Account Opening the issuance of LOE to BPI.

SECONDARY REQUIREMENTS
Acknowledgement of Asset Employee sign off on the AAF to confirm receipt of equipment and/or resources
Accountability Form issued to the employee

The following conditions shall apply relative to the timely and complete submission of mandatory pre-
employment requirements.

PRIMARY REQUIREMENTS

1. If the SSS, PhilHealth, PagIbig and TIN numbers are not submitted prior to the start date, I understand
that I will not be provided with a Letter of Endorsement (LOE) for BPI account opening. As I have been
made aware that my mode of payroll will be thru Bank of the Philippine Islands (BPI), I understand
that failure to secure my BPI Payroll account may cause delay with the processing of my salary, and
that the company shall not be held liable for any such delays due to my failure to comply with this
requirement.
1. If the NBI Clearance is not readily available, I agree to submit my NBI Appointment Schedule and
Receipt photocopy, OR Original Police Clearance, while the NBI Clearance is under process. I
understand that I am still required to submit the original copy of my NBI Clearance within one (1) month
from my employment start date.
2. If the School Records (Transcript of Records and Diploma) are not readily available, I agree that the
company is giving me (1) month from my employment start date to submit these mandatory
requirements. Failure to do so may result in the re-evaluation of my employment, tantamount to
disciplinary action or termination of employment.
3. I understand that my failure to comply with the PRIMARY REQUIREMENTS may result in the
withdrawal/cancellation of my Job Offer and/or may affect my employment with the Company,
including but not limited to the issuance of disciplinary action up to termination.

SECONDARY REQUIREMENTS

1. I am required to acknowledge the Asset Acknowledgement and Accountability Form (AAAF) to signify
that I have received the equipment and/or resources provided to me that are essential to the
performance of my job, and that I agree that I shall take full responsibility and accountability to these
items under the course of my employment with the Company.
2. I shall attend the New Employee Orientation (NEO), and comply with the new hire requirements, e.g.
online and/or face to face training.
3. I understand that my failure to comply with the SECONDARY REQUIREMENTS may affect my
employment with the Company, including but not limited to the issuance of disciplinary action up to
termination.
