Letter to Parents / Permission Slip

Dear Fourth Grade Parents,

In school we have been learning about animals’ traits, adaptations, life

cycles and anatomy! Our class has the opportunity to travel to the Imagination

Station in Toledo Ohio to participate in a Think Tank Workshop, where they will

have the opportunity to dissect a cow eye and get a better idea about the

anatomy of this vital organ. We are excited for this opportunity and will be taking

a field trip on November 3, 2021 from 8 -2 in order to explore the museum as

well.

The cost per child is $13.00, this includes the admission price of $8.00 and

the workshop price of $5.00. We would love to have at least 5 chaperones to

accompany us so we can form groups of 3-4 students per adult. The adult ticket

price is $14.00, and you must have a background check on file with the school in

order to participate. Those interested can email me at Kmarry@sienaheights.edu

to see if they have the appropriate paperwork filled out in order to volunteer.

Lunch will take place at 11:15 and can either be packed or purchased at the

atomic café. If lunch is being purchased students will need additional money to do

so as well as money to spend in the gift shop at the end of the trip if they wish.

Lunch is eaten in the tunnel between the imagination station and 5/3 building.
The address for the museum is 1 Discovery Way Toledo, Ohio 43604. If you

would like to research this trip on your own you can find more information at

imaginationstationtoledo.org, by calling their phone number 419-244-2674, or by

reaching out to me with any questions. Permission slips and the admission/think

tank workshop money are due by November 1, 2021 as well as chaperones

admission and background check approval. Lunch and giftshop money can be sent

with your child the day of the trip but will be your child’s responsibility to hold

onto.

Thank you for allowing me to teach your child as well as take them on this

knowledge filled expedition!
