AND SUBCUTANEOUS

INTRADERMAL INJECTION
For intradermal administration, Patient S was given ampicillin medication
on her left forearm. Patient S did not exhibit any allergic reaction/s after 30
minutes of performing allergy test.

INTRADERMAL INJECTION
At exactly 9:00am, Patient J received an injection of ampicillin on his left
forearm. This drug is administered for the purpose of drug allergy testing, thus it
only requires small amount of the said drug. Upon administering the drug, the
patient's skin surface developed a wheal or also called bleb which is a good
indication that the skin testing is successfully done. The student nurse encircled the
location where the bleb developed and indicated the potential end time of the
observation. The site was required to be observed for any adverse drug reactions
specifically, allergic reactions for the next 30 minutes (9:30am). Following
administration of the drug, redness was seen at the injection site. However, after
few minutes, redness around the injection site and the bleb gradually deflated and
disappeared within the time of observation. During which, the patient did not
complain of any form of itchiness, pain or hardening. After 30 minutes, finally,
there is no sign of drug allergy observed.
SUBCUTANEOUS INJECTION
Patient J received a subcutaneous injection containing sodium chloride in his
belly, 2 inches away from his belly button. This method of drug administration is
for medications that must be absorbed into the bloodstream slowly and steadily,

INTRADERMAL INJECTION
Patient X was injected with Antibiotic Ampicillin on his right inner forearm
at 11:07 am. After 30 minutes at 11:37 am, the injection will be subjected to
evaluation for any allergic reaction.
At the administration of medication, wheal or bleb was created. The patient
showed no sign of redness however, the patient reported minimal pain at the site of
injection. At 11:20 am, the bleb or the wheal was completely deflated. There were
no signs of redness and swelling nor the patient reported any itchiness or pain. The
medication showed no adverse and allergy reaction to the patient.
SUBCUTANEOUS INJECTION
Subcutaneous injection allows for slower absorption of a medication
compared with either intramuscular or intravenous site.
Patient X was injected with 0.5 mL of sterile water on his right lower
quadrant, 5 cm from his umbilicus, at 11:10 am. Patient X exhibited no signs of
swelling or redness after receiving the drug, nor did he display any negative side
effects or adverse reactions.

Name: Patient X
Patient X was successfully injected under the layer of the skin of his right
forearm with a small amount of ampicillin medication for allergy testing. At 11:52
am, the medication was administered, a bleb or wheal in the area/site was
positively formed or appeared. Patient X was being observed for 30 mins for any
allergic reactions to the patient’s skin.
After 30 mins of the medication being administered which is 12:22 pm, the
wheal or bleb was flattened. There were no signs of itchiness, redness, and any
immerse pain as reported by the patient. After an hour, the bleb or wheal has
completely disappeared.
SUBCUTANEOUS INJECTION
At 12:00 noon, Patient X was successfully injected with 0.5 of plain normal
saline solution on the lower left quadrant of his abdomen. When the PNSS was
being administered there were no adverse reactions like redness, swelling, and any
pain to the patient’s injection site and its surroundings.

Name: Patient C
Patient C has given or has been injected with 0.1 mL ampicillin medication
on her right forearm at exactly 8:53 am to be observed until 9:23 am, 30 minutes
after the medication administration for any drug reaction such as allergy within the
medication. The medication administration created a wheal/bleb which indicates
that the procedure done is right. Redness on the site was observed after
administering the injection which is a normal sign.
After 30 minutes of medication administration, there is no sign of itchiness
within the area and there is no sign of pain in the area. Aside from this, the bleb
also subside which indicates that the patient has no allergic reaction with the
ampicillin.
Name: Patient Y
INTRADERMAL INJECTION
Ampicillin medication was administered to Patient Y in her right arm at
10:21 am. It is due 30 minutes after the administration until 10:51 am and is under
observation for allergic response. A "wheal/bleb" was created at the skin's surface
when the medication was injected into the patient. Redness on the injection site
was observed after the drug had been injected into the patient.
During the 30 minutes, the patient verbalized that she felt pain in her right
arm, at the wheal, and on the injection site area. The redness on the injection site
was not subsiding as well as the bleb, and the wheal was not deflating.
Thirty minutes after the administration of the medication, the wheal was still
visible, and the bleb was hard. Redness was still present, and there was a physical
effect, which is a sign that the patient is allergic to the Ampicillin medication.
SUBCUTANEOUS INJECTION
Patient Y was injected with Sodium Chloride in the lower left quadrant of
her abdomen at 10:30 am. Redness was not observed on the injection site, and
there were no side effects upon administering the drug.

Name: Patient X
INTRADERMAL INJECTION
Intradermal injection is done to check whether the patient has an allergy to a
certain medication and it is also used for Tuberculosis tests.
Ampicillin was injected into Patient X at 11:35 a.m. on her right forearm and
due after 30 minutes upon the injection. At exactly 12:05 a.m. the injection will be
examined for any allergy reaction. When the drug was given, a “wheal or bleb”
formed on the surface of the skin.
30 minutes prior to injection the wheal or bleb slowly deflated and subsided
itself and patient x did not complain for any itching or pain, there was no
hardening happened and redness is totally gone which indicates that the patient has
no sign for drug allergy reaction.
SUBCUTANEOUS INJECTION
Subcutaneous injection is injected into the fatty tissue, just under the skin.
This route allows drugs to be absorbed slowly over a period of time.
At 11:40 a.m. patient x was injected with 0.5 ml of sterile water on her left
lower quadrant, 2 inches away from the umbilical area. Patient X showed no sign
of swelling or redness after injecting the drug and showed no side effects or
adverse reactions.

INTRADERMAL INJECTION
Patient X was injected directly with small amount of ampicillin medication
under the skin surface on the left forearm at 11:07 am. After the intradermal
injection, a bleb (small blister) or bubble-like area appeared. The patient was under
observation for 30 minutes for any possible reactions such as redness or swelling.
At 11:37 am, the wheal was completely deflated. There were no signs of
itchiness and immense pain. However, an area of slight redness was developed at
the site of injection. After an hour, it disappeared and was totally gone. The
medication showed no adverse and allergy reaction to the patient.
SUBCUTANEOUS INJECTION
At 11:42 am, Patient X was injected directly with 0.5 mL of sterile water on
the left lower quadrant of the abdomen. There were no signs of immense pain,
swelling or redness after the sterile water was administered. Patient X showed no
adverse reactions and side effects.
