The undersigned conducted validation of various establishments located at Tabuk

City, Kalinga pursuant to the permitting requirements of R.A 8749 ( Philippine Clean Air Act
of 1999) and R.A 9275 (Philippine Clean Water Act of 2004) on October 18 and 26, 2022.

FINDINGS AND OBSERVATIONS:

1. The Table shows the lists of surveyed establishments and the summary of our findings
and observations including geo-tagged photos.

of bigger septic tank and sealed the old septic tank.
3. Sources of water are from deepwell and Tabuk Water.
