AFFIDAVIT OF DESISTANCE

I, JOHNSON BRAVO, Filipino, of legal age, presently residing at

____________________________________________________ after having been sworn to in
accordance with law, do hereby depose and say, that:

1. I am one of the private complainant in the above-entitled case;

2. By way of mutual agreement with the respondents, I had agreed to amicably settle
the said case;

3. As a consequence of which, I received from the respondents the amount of

______________________), and hereby waives my right to any other claim
against the respondents based on the decision rendered in the above-entitled case;

4. I further acknowledge that after execution of this document entitled, “Affidavit

of Desistance”, I no longer have any lawful claim or claims of any kind and
nature against the respondents GEUEL HANDICRAFTS and MR.
SALVADOR BERNARDO JR./ SALVADOR BERNARDO SR., AND ITS
AGENTS, SUCCESSORS, AND ASSIGNEES, on account of the above –
entitled case covered by this “Affidavit of Desistance”;

5. Accordingly, I hereby remised, released, and forever discharge and by these

presents do for myself, my heirs, successors, and assignees remise, release and
forever discharge GEUEL HANDICRAFTS and MR. SALVADOR
BERNARDO JR./ SALVADOR BERNARDO SR.,, AND ITS AGENTS,
SUCCESSORS, AND ASSIGNEES,, from any and all manner of action or
actions, damages, claims and demands whatsoever, law or which, I, my heirs,
successors, and assignees hereafter can, shall, or may have, upon or by reason of
any matter, cause, or thing whatsoever, up to the time of these presents; the
intention hereof being to completely, absolutely, and finally release GEUEL
HANDICRAFTS and MR. SALVADOR BERNARDO JR./ SALVADOR
BERNARDO SR.,, AND ITS AGENTS, SUCCESSORS, AND ASSIGNEES,
 from all liabilities arising wholly or partially, directly or indirectly, on account of
the above –entitled case covered by this “Affidavit of Desistance”;

6. That I hereby agree that this AFFIDAVIT OF DESISTANCE may be pleaded in

bar to any suit or proceedings which I or anyone in my behalf may take against
GEUEL HANDICRAFTS and MR. SALVADOR BERNARDO JR./
SALVADOR BERNARDO SR.,, AND ITS AGENTS, SUCCESSORS, AND
ASSIGNEES, from all liabilities arising wholly or partially, directly or indirectly,
on account of the above –entitled case covered by this “Affidavit of Desistance”;

7. That this Affidavit of Desistance is being executed of my own free will and
volition and that no threats, force, intimidation or undue influence have been
made and/or exerted upon me by any person;

8. I hereby certify that we understood the contents and importance of this Affidavit
of Desistance.
