WITNESSED: That-
WHEREAS, the LGU has agreed to accept the SCHOOL’s qualified OJTs student-
trainees to undergo job training in their premises;
, who intend to conduct practicum exposure in the LGU as part of the SCHOOL curriculum;
2.The HEI shall provide the student-trainee, the basic orientation on word values,
behavior, and discipline to ensure smooth cooperation with the LGU;
3.The HEI shall issue an official endorsement vouching for the well-being of the
student-trainee, which shall be used by the LGU for processing the application of the student-
trainee;
4.The HEI shall voluntarily withdraw a student-trainee who is found to misbehave
and/or act in defiance to existing standards, rules and regulations of the LGU and impose
necessary university sanctions of the student-trainee;
5.The LGU, may grant allowance and/or adequate insurance to the student-trainee, in a
accordance with the LGU’S existing rules and regulations;
6. The LGU, upon consultation with HEI may require qualified students to submit
themselves to examination, interviews, and file pertinent documents to support their
application;
7.The student-trainee shall be fully vaccinated and follow the safety protocols
implemented by the local Government;
a. Always bring vaccination card;
b. Wear facemask at all times and face shields if necessary;
c. Have their temperature checked;
d. Ensure physical distancing;
e. Shall provide an adequate number of hands washing or hand sanitizing.
8.The student-trainee is required to complete the minimum but not limited to 320
training hours;
9.There is no employer-employee relationship between the LGU and student-trainee;
10. Any violation of the foregoing covenants will warrant the cancellation of
Memorandum of Agreement by the LGU within thirty (30) days upon notice to HEI;
11. This Memorandum of Agreement shall be effective upon signature of both parties
and implementation will begin immediately and shall continue to be valid hereafter until
written notice is given by either party, thirty (30) days prior to the date of intended
termination.
