1.0 PURPOSE
The intent of this document is to provide a complete Method Statement for the use of materials,
equipment, tools and plants; it includes the manpower and construction methodology in compliance
with RC requirements, drawings and technical specification with safety measures and quality control
2.0 SCOPE OF WORK
This method statement sets out requirements for the fabrication, erection and stripping of formwork for
concrete works. The scope of works is to provide guidance to prevent injury to persons engaged in
erecting and dismantling formwork and associated equipment as per RC Guideline and Specification
(RC GLS).
Refer Appendix-3 for the list of sites where we intend to use the formwork accordingly.

Material handling and delivery on site


Materials will be delivered and received in the lay down area store room, in secured and controlled
room, or temporary lay down area at site. For the ready mix concrete it will be deliver on site by
transit mixer and it will be poured using a concrete pump.

5.0. SITE PLANNING AND PREPARATION


Encroachment permit will be obtained first before starting any activity in the site. Once the area is
determined and staking is complete, the safety personnel will established the perimeter barricade to
warn others. If rerouting is necessary the safety personnel will establish first the rerouting of access
roads and install warning devices to warn passing traffic in the area and they will also coordinate with
other contractors working in the area for proper coordination. The safety personnel will coordinate
with the construction supervisor for the entry and positioning of heavy equipment and establish a
danger zone so that safety personnel will install warning signs. Prior to excavation using heavy
equipment a manual excavation shall be done first if the area is a high risk area (presence of
underground facilities), once all underground facilities is secured then equipment excavation can be
implemented. All communication in the site will be by phone and constant supervision will be
implemented during the excavation in order to avoid untoward accident.
Use Cable detector in case the area is having cable beneath the excavation. All as built drawings must
be refer to ensure the presence of cables/utilities.
6.0 WORK PROCEDURE
6.1 PREPARATION
6.1.1 Formwork for concrete shall be constructed to meet the requirements of the structure and
the required finish of the concrete. It shall be constructed in a way that the concrete can be
properly placed and strong enough to withstand the load and shall conform to the required
shape, dimension, level, line and position while at use.
6.1.2 Formwork shall be strong, stable and rigid to withstand the pressure of fresh concrete
during the pouring and to keep or maintain its position and shape. The number of uses of
any particular formwork shall be such that the quality of the specified finish is maintained.
Formed surface shall be smooth finish unless otherwise noted. Wood, generally laminated
plywood and good quality timber, shall be used for formwork in this project.
6.1.3 Form ties to be used in securing and maintaining the forms to position shall be tie rods,
clamps, form jacks or wood, or any combination of which that is suitable depending upon
site condition.
6.1.4 Forms shall be sealed so as to prevent leakage particularly from the bottoms and sides.
6.1.5 Forms shall be cleaned of all debris and unwanted materials prior to pouring.
6.1.6 Forms shall be applied with a thin and uniform layer of “Form Release Agent” prior to
placement at locations or may be applied in – situ; but care shall be taken to allow sufficient
time to dry before pouring of concrete.
6.2 INSTALLATION

6.2.1 All dimensions are related to finished concrete. Other details shall be in accordance with
the appropriate provisions of ACI 301 / ACI 318, using either 12mm or 18mm thick marine
plywood and ribbed with 2”x4” timber. Form frame shall have longitudinal ribs, and ribs
shall be free from deformities and defects to resist buckling effect when subjected to
enormous dead load of concrete.

6.2.2 Fix 25mm plastic chamfer strips to exposed exterior corners of the concrete structure or as
specified on the construction drawing.
6.2.3 Assemble and fix the forms to the correct position as marked by the Surveyor. Check the
dimension to conform to the required size and shape of the concrete member, subject to the
tolerances prescribed in the project specification.
6.2.4 Secure the forms to its position properly by using tie rods, clamps, or wood cleats or
combination of which that is suitable depending on site condition. Forms must be watertight
to prevent run-off of cement paste from the fresh concrete and to avoid honeycombs.
6.2.5 Wall forms shall be in layer of sectional units. The joints formed by the assembly of these
sectional units must be fitted well, aligned and secured properly.
6.2.6 In the case of overhead formworks, make sure that the scaffolding and other part of the
staging are properly fixed and have passed the safety requirements.
6.2.7 Provide bracings on sides of forms to prevent any movement or displacement. For overhead
formwork, bracings can be anchored on part of the scaffold found suitable.
6.2.8 Check verticality by attaching plumbs on the sides of the forms or by using spirit level.
6.2.9 Forms for rectangular and square shaped structures shall be maintained at right angle.
6.2.10 Mark the top of concrete elevation by fixing nail on two opposite sides inside the form and
on starter bars for wider areas of pouring.
6.2.11 Clean the inside forms. For deep forms, provide an opening at the bottom of the formworks
big enough for dirt, small wood pieces and other debris to pass through when air blown.
6.2.12 Deep formwork shall have a cut out / window opening at every 2.0 meter and side chutes or
provide extension to boom of concrete pump like flexible hose or funnel to eliminate higher
drop of concrete and concrete segregation.
6.2.13 Forms shall be free of noticeable wrinkles, warps, big patches, and other such defects.
Contact surface of forms shall be clean, and free of ruts and other roughness.
6.2.14 Determine the wind velocity during installation of wide and big pre-fabricated formworks
and stop when the wind speed dictates that it’s unsafe to do lifting works.
6.2.15 Water stops shall be polyvinyl chloride material and fixed at necessary locations based on
the approved drawings.
6.2.16 Water stops shall be made continuous by heat welding and corners also shall be mitered.
Water stops shall be nailed into forms, using wood strips if necessary.

6.3 REMOVAL OF FORMS


6.3.1 Forms shall be removed in such a manner as to ensure the complete safety of the structure.
6.3.2 Remove the bracings and supports carefully to avoid inflicting or causing damage to the
concrete.
6.3.3 Use a crowbar strip and remove forms carefully in such a manner that will not cause any
damage to the concrete surface.
6.3.4 Remove the tie rods. The tie rod holes shall be filled up with approved patching material.
6.3.5 Clean the removed forms of any hardened concrete. Remove any protruding nails, wood
and plywood spurs upon removing the formwork.
6.3.6 Coat all forms with painting compound in accordance with manufacturer’s
recommendations. Minimum times between placing concrete and removing forms shall be
as follows, unless otherwise justified

Columns, walls and sides of beams and girders – 2 days.


Slabs and bottom of beam and girders – 7 days.
6.3.7 In cold weather conditions (air temperature below 10°C) minimum time before removing of
forms shall be doubled.
6.4 SITE CLEAN-UP AND RESTORATION

6.4.1 Properly dispose the demolished materials. Demolished material shall not be allowed to
accumulate on site.
6.4.2 Remove and transport debris and rubbish in a manner that will prevent spillage on existing
canal adjacent to the working area.
6.4.3 Leave site clean, neat and order in good condition.
6.4.4 Remove all tools used, equipment and materials from the site upon completion of work.
6.4.5 Level site to required elevation and profile and clean the area.
6.4.6 Clean and repair damages caused by installation and construction activities used for
temporary works.
6.4.7 Restore existing and permanent facilities used during construction to original or better
condition.

6.5 SITE WELFARE

Welfare facilities on site shall be provided like rest areas, toilet or washrooms, shaded area,
chilled water dispensing area, smoking area, prayer area.
6.6 ORGANISATION, RESPONSIBILITY & SUPERVISION

Construction Manager – will be the one to plan and coordinate all the activities in the site
and will be responsible for coordinating with all other departments as well.
Safety Personnel – will be the one to establish all necessary warning signs and barricades
establish the high risk areas in coordination with the Construction Manager and constantly
monitor the activity in terms of safety to all the workers. Furthermore he will give advice on
current and forecasted safety issues and concern.
Site Engineer - will be the one to implement the instruction of the Construction Manager in
the site, he will also directly supervise constantly the manpower and equipment working in
the area and implement all safety regulation and precaution set by the safety personnel.
Quality Control Inspector – Will be the one to inspect and monitor the area before
recommending to RC Engineer for inspection through RFI. (Request for inspection)
Safety Manager – Will be responsible for managing day to day activities and will identify
hazards, evaluate risks, advise safe and best practices for the safety of all personals.
Safety Officer – will report directly to safety manager, and will assist him to organise the site
as per RC approved procedures
Supervisor – will be the one to supervise and assign the works as per schedule.

Carpenters – will be the one to install and remove the shuttering.

6.7 WORK FORCE

7.0 HEALTH AND SAFETY

7.1. RISK ASSESSMENT


Detailed Risk assessment for all activities is provided in the Appendix-2
7.2 ENVIRONMENTAL SAFETY & HEALTH CONTRACTOR REQUIREMENTS
Section II series 15 (Personal Protective Equipment)
 Concrete wash pits will be provided at site for the disposal of concrete.
 Skips will be provided at site for garbage collection and will be disposed of properly.
 Spill tray will be provided for the genset or compressor in order to avoid oil spill.
 Chemical storage area will be set in accordance with RC requirement.
 Spill kit will be kept at site
7.3. PROTECTIVE AND SAFETY EQUIPMENT
 Hard hat, to be worn at all times
 Safety shoes, to be worn all times
 Safety gloves, to be worn all times
 Safety goggles, for workers around working equipment- (Tinted type for outdoors).
 Full Body Harness with Double Lanyard
 High visibility vest, especially for workers around machinery, to be worn all times
 Ear plugs & Ear Muff.
 Mask(N95 Dust Mask)
7.4. CONTRACTUAL REQUIREMENT
 SECTION II – 200 SERIES (15: Personal Protective Equipment):
15.1 Personal Protective Equipment shall be considered only as a “last resort of defence” against
potential risks and hazards. Where so ever possible hazards and risk are to be addressed and
reduced by other means.
15.2 based upon the findings of 3.1.3 Section 1 and 11 above the contractor shall ensure that
adequate stock of Personal Protective Equipment are readily either available on site or issued to
personnel. PPE shall be considered as consumable items.
15.3 All PPE utilised shall be manufactured to a recognized international acceptable standards

7.5. INFORMATION TO PERSONNEL


All the manual workers that will be working in the site will be informed and reminded by
conducting morning tool box meeting in order to continuously remind them of the existing safety
risk and it will be handled by the designated site supervisor and sometimes with the presence of
the safety personnel.
7.6. EMERGENCY PROCEDURE
The following are the safety procedure that will be implemented in case of accident.
 In case of traffic accident or person hit by the equipment notify immediately the police and
emergency crew do not engage in moving the person who are injured.
 Emergency contact numbers will be prominently posted on bulletin board.

 First Aid box will be readily available on site.


7.7. PERMITS REQUIREMENT
The following are the required permit before implementing the activity:
 Encroachment permit
 Work permit (Excavation permit & Hot work permit if any)
 Transport Permit

8.0 ENVIRONMENTAL ISSUES


8.1. ENVIRONMENTAL IMPACT ASSESSMENT
 One possible risk to the environment is oil spillage may be due to equipment breakdown. If
and when this will happen immediately contained the area of contamination then remove
the contaminated soil. In order to avoid this incident proper maintenance procedure for the
equipment shall be implemented.
 Other possible risk to environment is garbage and debris accumulated thru-out the
construction activity, for this matter collection bin shall be used and proper disposal and
housekeeping shall be done.
 Other possible risk is water may overflow flow from channel or canal, should not be
disturbed, when disposing excavated soil, washing of vehicles and equipment.
 Excess concrete will be dumped in RC designated dumping areas.
 In case of minor or major spill into water channel, there is possibility of contamination of
water channel flowing into sea; competent environmental personnel will be brought for
advice.

9.0 QUALITY CONTROL


9.1. CONTRACTUAL REQUIREMENTS
 Prior to the construction activity RC Engineer shall inspect the area to verify the site
conditions.
 RC Engineer shall inspect the approved materials which will be incorporated into works.
 RC engineers shall inspect hold point release (HPR) and witness points as per ITP.
9.2. CRITICAL WORKS
The possible critical work for this activity is during the excavation & concrete pouring,
could be the possible cave-in of excavated area due to softening of sub-soil or erosion due to
water seepage and the presence of concrete pump & mixer during concrete pouring
activities.
9.3. INSPECTION AND TESTING
Inspection of Formwork shall be done as per the approved Inspection & Test Plan (ITP).
Refer Appendix-1 for ITP.
9.4. REQUEST FOR INSPECTION (RFI)
Request for Inspection of Work (RFI) shall be issued to RC Field Engineer 24 hours in
advance. The pre-concreting RFI is raised once the formwork is completed.
9.5. FOLLOW UP AND EVALUATION
We intend to follow up and evaluate our performance by constantly implementing strict
quality control as this is the most critical activity that can affect our progress and constantly
monitoring it and implement measures to expedite this activity. The crewing system shall be
monitored constantly in order to meet the required volume installed and any delays shall be
recorded and evaluated in order to implement measures and improvements to augment the
productivity.
9.6 BEFORE CONCRETING
9.6.1 The top level of concrete must be marked on formwork using total station.
9.6.2 Use total station / hand tools or line plumb tool to check formwork verticality or movements
before concreting.
9.6.3 Wedges uses for final alignment before concrete placement should secured in position before
the final check.
9.6.4 Formworks must be anchored to the shores below so that movement of any part of the
formwork system will be prevented during concreting.
9.6.5 Additional elevation of formwork should provided to allow for closure of form joints,
settlements of mudsills, shrinkage of lumber, and elastic shortening and dead load deflection
of formworks.
9.6.6 Positive means of adjustments (wedges of jack) should be provided to permit realignment or
readjustment of shores if settlement occurs.
9.7 DURING AND AFTER CONCRETING

9.7.1 Before the initial set of the concrete, the elevations, camber, and plumbness of formworks
systems should be checked, using total station or any other device or tool.

10.0 SAFETY ACTION PLAN

10.1 Saudi Withcon Co. Ltd., safety supervisor shall carry out the safety site inspections to
ensure that safety requirements laid out in the safe work plan are followed during and after
the concrete works. Safety Manager / Supervisor shall review the Safety Action Plan and
secure Work Permit if necessary prior to start of any activities.
10.2 Barricading “swing” areas of heavy equipment shall be checked before work begins.
10.3 Equipment, tools and materials inspection.
10.4 All equipment for concrete works shall have to be inspected by Saudi Withcon Co. Ltd.,
Safety Manager / Supervisor prior to use at site to ensure their safe worthiness at work.
10.5 Enough working spaces will be provided and the enter/exit access. Proper barricading and
safety signboards, emergency equipments introduced. Designated safety person shall
monitor and act throughout the whole work activity.
10.6 During concrete works, first the area shall be under safety work area for the whole duration
of the activity. All the precautionary measures shall be taken considering the probability of
emergency. In hospitalization cases, ambulance will be provided.
10.7 Emergency exit point, assembly area, emergency evacuation plan and emergency numbers
shall be posted on the boards
11.0 CONFIRMATION OF INSTRUCTIONS TO OPERATIVES

Those names listed below confirm receipt of the method statement and have had the contents explained to
them/understand it and are aware of the site ES&H requirements.
