NCM 104 COMMUNITY HEALTH NURSING

NARRATIVE REPORT

I am currently taking a degree of Bachelor of Science in Nursing, and one of the


subjects we are required to accomplish is the Community Health Nursing. We were
assigned or tasked to conduct a home visit in different barangays or in our barangay.
We were tasked to have an interview with the family and assess their needs, weakness,
and deficiency.

As a nursing student who wants to learn and succeed, I obeyed what has been
assigned to us. I conducted a Home Visit here in our barangay, Barangay La Opinion,
Nabua, Camarines Sur. Since I am familiar with the family that I visited, it was calm and
no pressure when I was talking with the mother.

Prior to visiting them, I already inform them that I will conduct home visit and will
ask questions. They gladly agree and set the schedule where it is convenient for both
parties since I have class and they are also busy. We agreed to meet last December 11,
2022.

I knocked on the door and  greeted them as I entered their home and then I
began to interview them. I began by explaining to them why I had come to see them. I
continued by inquiring about the family's leader, husband, and other members at that
point. The mother patiently responded to my queries, providing details on her children's
age, birthday, level of education, job, and other answers. Furthermore, when I asked
them if they belonged to NHTS like 4Ps, the mother answered yes When I asked the
mother if they have PhilHealth, she told me yes but her husband is the owner and they
are just beneficiaries.

Moreover, regarding their vaccinations, they are all vaccinated for COVID-19
except to their son who is 10 years old. I asked the mother, “Why?” she replied, “I’m
afraid what might be the side effects since he is still a kid. But I am planning to get him
vaccinated one of these days.” I am happy with her answer but I still gave her further
teaching about the importance of vaccines that will help all of us not to get sick.

When I asked the mother if they are currently using any form of family planning,
she said no. Their reason is that, they want to have another child since their eldest is 10
years old now and they want to have 2 to 3 children. To top it all, they already set plan
for their desired family and using artificial or natural way of family planning is not their
top priority as of now. But again, I implemented health teaching to the mother of the
importance of family planning.

For their livelihood, They are living near a irrigation and they have land and they
own it. They also have herbal plants in the backyard of their house such as tanglad,
lagundi leaves, and other herbal plants. The mother said this herbal medicine has been
their partner for years when they are getting sick such as cough, colds, and fever.

The mother verbalizes that her children are fond of playing near the irrigation. There are
Low-lying mosquitoes frolicking in the vicinity.The mother hasn’t been using insect
repellant. Old tires and unmoved pails which are both uncovered and filled with water
that’s why his child had dengue.

To sum up everything, the family knew their weaknesses and needs but they
need further health teachings like the vaccines, family planning, and health teaching
regarding dengue fever. Although I gave them enough information, I want to know if

they completely understand all of it. Thus, further teaching and follow-up visit should be
implemented.
