Anniversary on January 24, 2023 (Tuesday), from 5:00 pm to 7:00 pm at YASC Covered
Court.

WAIVER FORM

I am aware of the nature of the activity and find it essential in my role in the
YASC school community. I also understand that adequate and appropriate supervision
will be provided.
I allow my son/daughter to participate in the scheduled activities. We recognize
that unanticipated situations and problems can arise during the activity. And so, we
further agree that YASC will not be liable for any untoward incident that may take place.
