Pipeline Rupture from Bulldozer Impact
What happened?
Heavy equipment operators were clearing sand from a
Impact:
pipeline corridor right of way (ROW). A bulldozer struck a ▪ Contractor fatality
crude pipeline, causing a rupture and hydrocarbon release ▪ PSE Tier 1 hydrocarbon release with extensive fire
which caught fire and resulted in a fatality.

What went wrong?

• Equipment operators were not aware of the presence and
process safety hazards of the buried crude oil pipeline
• No work permit was issued for ROW clearing
• Deficient monitoring of contractor safety
• Design of the ROW did not consider nearby oil pipeline 1 meter
• Infrequent pipeline monitoring to address ongoing non-
permitted activity in the pipeline corridor
• Delayed incident notification and leak detection alarm
systems that did not alert resulted in extended time to
achieve safe isolation
• Land Use Permit (LUP) requirement for ROW spacing was
not followed
