Since, these pupils were supposed to be enrolled under Special Education (SpEd) due to their conditions.

1. Scanned Endorsement Letter signed by the school head stating the issue/s and details (inlcude
name of learner/s)
2. Duly filled-up RF in excel format
3. Scanned copy of the duly filled-up RF (with signature of SH)
4. Scanned copy of Birth Certificate, if needed
5. Scanned copy of Form 137 (SF10) or Form 138 (SF9), if needed
