WARNING LETTER

Dear Mr.

This letter serves as a formal warning for your poor performance from the past few days.

It was observed by the management that your performance in the past few weeks has not been
satisfactory on occasions such as follows:

You have been advised from time to time to improve upon your performance however we regret to
inform you that we did not find any improvement.

Management would have been justified in taking serious disciplinary actions against you due to not
satisfactory performance. It is to be noted that based on the Labor Code of the Philippines Article 287
section 282 than an employer may terminate an employment due to the following causes:
a) Serious misconduct or willful disobedience by the employee of the lawful orders of his employer
or representative in connection with his work;
b) Gross and habitual neglect by the employee of his duties;
c) Fraud or willful breach by the employee of the trust reposed in him by his employer or duly
authorized representative;
d) Commission of a crime or offense by the employee against the person of his employer or any
immediate member of his family or his duly authorized representatives.

However for this time, the management has taken a lenient view and decided to warn you.

You are advised to note that any repetition of such or other act on your part in the future will be dealt
seriously.
