AFFIDAVIT OF WAIVER OF RIGHTS

I, JOHN PATRICK FLORENDO ECHALAR, of legal age, single, Filipino and a

resident of Miranda Compound Phase III Brgy. Maimpis City of San Fernando, Pampanga, after
having been duly sworn in accordance with law, do hereby depose and say that:

1. I am the member-beneficiary and owner of the rights over a certain lot situated at Block 1
Lot 35 of Pinag-isang Magkakapitbahay Ng Miranda Compound HOA Inc. Phase III with
address at Maimpis City of San Fernando, Pampanga containing an area of Forty Nine
(49) square meters under the Community Mortgage Program (CMP) of Social Housing
Finance Corporation (SHFC);

2. Considering the fact that I am no longer interested to the said lot and has failed to comply
with my financial obligations and requirements of the Homeowners Association HOA
Inc. Phase III, I desire to transfer the same to another person;

3. As such, I am waiving all my rights, interest and participation over the same in favor of
ROSEMARIE O. ARANAS, likewise of legal age, married and with residence and postal
address at SMC Boulevard Purok 1 Quebiawan, San Fernando, Pampanga;

4. The said ROSEMARIE O. ARANAS is willing to assume all obligations with the HOA
and with SHFC in connection with the above-mentioned lot;

5. I am executing this Affidavit to attest to the truth of the facts aforementioned; and

6. Further, I am executing this affidavit in compliance with SHFC Corporate Circular No.
16-046, Series of 2016 regarding the Amendments to the Guidelines on substitution of
MBs.
