Good Day.

I am ____, a licensed professional teacher with registration number ___. As a

professional teacher, I was issued with a PRC license. The said PRC license is
valid until February 17, 2023.

In relation to the validity of my PRC ID which will be expiring on February 17,

2023, I applied for renewal of my License via Leris. However, when I signed-in
using my first account under the e-mail address ____, I received a notification
stating, “record mismatch”. The mismatch may have been caused by the fact
that I am now using the last name, “___”, which is the last name of my
husband. Under the first account, I was still using the last name “____”. Hence,
I signed-up for a new account.

In the new account, I used the e-mail address ___ However, when I logged-in
using the new e-mail address, I received a notification stating “The account you
entered does not exist. Enter a valid e-mail or create a new account”.

When I tried to create a new account, I received another notification stating that
the name __ already exists. Thereafter, I was not able to access Leris to process
the renewal of my license.

In view thereof, I am requesting help from your office for the retrieval of my
account or any kind of help which will enable me to renew my PRC license.
