LETTER OF AUTHORIZATION

DEAR SIR/MADAM:

I, MARIVIC MACUTAY DE LEON, Filipino citizen, of legal age,

married, and with residence address at #144 Aquino Street, Annafunan East,
Tuguegarao City, Cagayan, do hereby authorize my daughter, MONIQUE
VIVIEN MACUTAY DE LEON, likewise of legal age, single, and with
residence address at #144 Aquino Street, Annafunan East, Tuguegarao City,
Cagayan, to receive or claim my passport from the Department of Foreign Affairs
– Tuguegarao (Regional Government Center) which has a tentative date of release
on 07 November 2022.

I am also authorizing her to make, sign and execute, for and on my behalf,
any documents which may be necessary for the release of the above-mentioned
passport.
