TESDA Representative : Good morning mam AC Manager, I will give you the 10 Rating sheets, 1 Assessors Guide, 10 Candidate Guide and
10 CARS that will be needed by our assessors.

(Assessor will enter and will show the appointment letter to the AC Manager.)

Assessor: Mam Ac Manager! Here is my appointment letter that will serve as a proof that I’ am the assigned assessor for this assessment.
Assessor to AC manager: I would like to check the completeness of tools and equipment, if the assessment room is ventilated and illuminated and
safe:

Assessor to AC Manager : I checked that the tools and equipment are complete, the assessment room is well ventilated and illuminated and safe.
So we can now start the assessment.

AC Manager : These are the documents that you needed for this assessment here is the 10 Rating Sheets, 1 Assessors Guide, 10 CARS,10
specific instruction, 3 attendance sheet, 10 filled-up self-assessment guide and 10 application form.

CANDITDATES PREPARATION (ASSESOR)

Good morning candidates, I am ____________________, your competency accredited assessor for _______________NC II. Here is my ID with 1234
number that will serve as proof or evidence.
Before we start, I would like to acknowledge the presence of our TESDA representative Mr/s. ____________ and our AC manager Mr/s.
________________. You do not have to worry about their presence. They are here to oversee the conduct of assessment to check if the conduct is
in accordance with the prescribed competency assessment methodologies and procedures. They will not assess you.

But before anything else, let me first check your attendance. As I call your name, please come forward and present to me your admission
slip. (Assessor will check and compare whether the candidate is the same person on the attendance sheet)
I have here with me accomplished Application Form and SELF ASSESSMENT GUIDE AND APPLICATION FORM. This is an indication that you know
and can perform all the units of competency enumerated for Computer Systems Servicing NC II. So before we start our assessment, let’s have a
brief orientation first.

What is Assessment?
-It is the process of collecting evidence and making judgment on whether competency has been achieved.
-What is the purpose of the Assessment? It is for Qualification, Recruitment, and Educational Needs.
NC?______________________. It is composed of Basic, Common and Core Competencies. But we will be focusing on the core units of
competencies, namely; Core1, 2, 3,4______________________

NC2? _________________________________ has two types of assessments, the Demonstration and Oral Interview.

IF COC CERTIFICATE OF COMPETENCY (LIKE CSS ,EPAS, EIM, BPP, ORGANIC, BREAD)

Computer Systems Servicing NC II is a COC type of Assessment In every COC, you have to perform every task with time allotment
In COC1, you have to Assemble computer hardware, prepare USB installer, and install operating system for 2 hrs
In COC2, you have to prepare the network cable, setup router and setup Wi-Fi for 2 and half hours
In COC3, you have to setup user access, setup domain and setup and deploy printer for 1 and half hours
Lastly, in COC 4, you have to back-up and restore computer system and repair computer for 1 hour
You will be given 7 hours to perform, your task and also you have 1 hour for your oral interview, a total of 8 hours.

In Oral questioning you need to answer at least 8 correctly out of 10 questions to be considered satisfactory.

For your assessment procedure, candidates number 1-10 should perform COC1 followed by CO2, COC3, COC4 for COC and activates if project type

We also have the DO’S and DON’TS:

DO’S
-Always follow the OHS procedure when conducting the demonstration.
DON’TS
-Use of cellular phone is not allowed
-Leaving the premises of AC without informing the assessor or staff, -Loitering in the premises.

We also consider allowable adjustments, Is there anybody has religious practices that need to address during the assessment, or taking any
medications to be considered? Is there anyone here have difficulties in understanding English language?

As a candidate, If you are not satisfied with the result given by me, you have a right to appeal through writing addressed to the AC manager,
immediately after assessment the assessment.

And me as an assessor I have the right to stop the assessment if there is a danger to occur.

Upon receiving your CARS which will be given to you by the AC manager, after 3 working days you may apply for the issuance of your NC at TESDA
Gaysano OSCO office just bring with you the following requirements:
a. CARS
b. passport size picture with white background and
c. php 50.00 for processing fee.

Your NC is valid for 5 years

If you receive COC or not competent, you have 2 options:
a. Apply for reassessment of your COC at any accredited assessment center in your own most convenient time. Or
b. You may apply for the issuance of your COC at TESDA Gaysano OSCO office

The result of this assessment is strictly confidential

(DURING THE ASSESMENT)

I will now assign your candidate’s number based on the attendance sheet. Here is for you Candidate number 1 2….. etc.

If you don’t have any questions, I will now give you the copy of specific instructions, please read them carefully for 10 minutes, and don’t hesitate
to ask questions. Okay, times up!

Now, let me introduce to you the tools and equipment that you will use during this assessment . . . these are the tools.

Ok, let’s start the COC1, Assemble the computer hardware, prepare the USB installer and Install operating system your time starts now . . . (bring
with you the rating sheet, checking and writing) okay, stop. Let’s proceed to the COC2, prepare network cable, setup router, setup Wi-Fi. . .Okay
start, okay stop. Let’s come to the COC3, setup user access, setup domain and setup and deploy printer . . . start now stop. Proceed to COC4, Back-
up and restore computer system and repair computer . . .Start, ok times up, For All candidate, go to waiting area and get ready for the oral
interview as I call your candidate number.

FOR COC:

Ok, so in your demonstration in COC1, you have assembled and installed the computer system, so you have a satisfactory rate.
In COC2 you have successfully set-up your computer network cable, configured your system device and configured the router, access point,
wireless and firewall, you’ve got satisfactory rate.
In COC3 you have set-up your computer server; you are successfully in your file sharing, remote desktop and printer sharing. You’ve got satisfactory
rate in COC3,
And in COC4 you successfully repaired and back-up your computer system and network do you get satisfactory in COC4. Your overall result for your
demonstration is SATISFACTORY I will sign (THE ASSESSOR WILL SIGN ON THE RATING SHEET) you sign(THE CANDIDATE WILL SIGN THE RATING
SHEET)

And your oral interview out of 10 question you’ve got 9 correct answer so you’ve got satisfactory rate in your oral interview, I will sign (THE

GET THE CARS FOR COC ….
For the Summary….
In COC1,COC2,COC3, and COC4 you’ve got satisfactory rate in your demonstration and interview (check the 4 demonstration and interview in four
competency) so, my recommendation is for the issuance on NC (encircle the NC and write the CSS NC-II ) for CSS NC-II, (check yes ) and your
competent (write the competent also in the bottom). (sign the assessor signature ) please sign here ( sign the candidate signature). Your CARS will
be released to you by your AC MANAGER immediately after the assessment, after 3 working days you may apply for the issuance of your NC at
TESDA Provincial Office just bring with the following requirement. CARS, passport size picture with white background, php 50.00 for processing fee.

Ok, so in your demonstration in COC1, you have assembled and installed the computer system, So you have a satisfactory rate.
In COC2 you have successfully set-up your computer network cable, configured your system device and configured the router, access point,
wireless and firewall, you’ve got satisfactory rate.
In COC3 you have set-up your computer, you are successful in you file sharing and remote desktop HOWEVER IT doesn’t print in printer sharing
WHICH IS CRITICAL ON THE ASSESSMENT . So you’ve not satisfactory rate in COC3, my advice to you is to practice installing and configuring the
printer. OK?

In COC4 you successfully repaired and back-up your computer system and network do you get satisfactory in COC4. Your overall result for your
demonstration is NOT SATISFACTORY I will sign (THE ASSESSOR WILL SIGN ON THE RATING SHEET) you sign(THE CANDIDATE WILL SIGN THE RATING
SHEET).

And your oral interview out of 10 question you’ve got 5 correct answer so you’ve got satisfactory rate in your oral interview, I will sign (THE

GET THE CARS ….

In COC1,COC2, and COC4 you’ve got satisfactory result, in your demonstration and interview (check the 4 demonstration and interview in four
competency) in COC3 you are satisfactory in interview however in COC3 your demonstration is not satisfactory so, my recommendation is for
issuance of COC 1,2,4(ENCIRCLE THE COC AND WRITE COC1,2,4 IN THE BLANK SPACE BELOW) and for reassessment of COC3 (CHECK THE
REASSESSMENT AND WRITE COC3 IN THE BLANK SPACE BELOW) I will sign (THE ASSESSOR WILL SIGN ON THE RATING SHEET) you sight(THE
CANDIDATE WILL SIGN THE RATING SHEET)

Your overall result is not yet competent (CHECK THE NOT YET COMPETENT AND NO IN THE CARS)

Your cars will be issued by assessment center manager after the assessment. And you have two option You may apply for the issuance of your COC
(COC and write COC1,COC2,COC4) at TESDA Gaysano OSCO office just bring the following requirement:
 CARS
 Passport size with white back ground
 50 php for processing fee
Or apply for re-assessment of your COC 3 at any accredited assessment center in your own convenient time. Ok

RECORD THE RESULT OF THE ASSESSMENT OF THE ASSESSMENT IN ATTENDANCE SHEET AND LET AC MANAGER SIGN THE CARS AND ATTENDANCE
SHEET. LET ALSO THE TESDA REP TO SIGN THE ATTENDANCE SHEET.

REPORTING OF ASSESSMENT TO MANAGER.MAM/SIR I HAVE HERE TO RETURN THE 3 copies of attendance sheet, 10 copies of application form,
and 10 copies of filled up self-assessment guide, 10 copies of candidates guide, 1 copy of assessor guide and 10 copies CARS.
