    CHANGE ORDER LETTER

    The purpose of this letter is to inform the body the changes made
    in the previous order by RHU-San Isidro. These are the following
    products that have been changed or requested by the receiving
    company:

    1. HYDROXYZINE 25MG TAB. To SYMDEX TAB.


    (Reason: Phased-out Item)
    2. DIAZEPAM 10MG/2ML VIAL To CEFUROXIME 500MG TAB.
    (Reason: Requires substantial paperwork for prescription and dispensing)
    3. BETAHISTINE 8MG VIAL To BETAHISTINE TAB.
    (Reason: Incorrect or typo error in the request order)
