Dear Sir,
I am Muthiah Sivarajan bearing Sri-Lankan NIC Number 194625801815 and Sri-Lankan
Passport Number N4708754 is writing this letter to obtain a new Sri-Lankan passport. The
current passport of mine is expiring on the 28th of January 2022. I got the appointment at
your office on the 9th of February 2023 to come apply for one but I currently reside and live
in India with my daughter and my son, I’m a heart patient and I solely came down to Sri-
Lanka to renew my passport. Soon after my arrival I got to know that there is a delay in
obtaining passports with the current situation.

I kindly request your good self to please do the necessary arrangements and issue me the
passport as soon as possible. I’m 76 years old and is unable to walk or stand in queues for a
very long time.

I’m appointing my nephew Nagaratnam Vijayakanthan (NIC No: 197318401667) to visit the
passport office on behalf of me and submit the application for my new passport.

I will appreciate if this can be accepted and completed as soon as possible as I have to go
back to India.

I’m attaching my medical documents regards to my health along with this for your kind
reference.
