OATH OF MASTER TO MANIFEST IN CLEARANCE FOR A FOREIGN PORT

I, CAPT. ISIDRITO U. LAGOS Master or commander of the MV “AQUASALWADOR” bound

from the port of MANILA, PHILIPPINES to ___________________________ sincerely
and truly swear (or affirm) that the manifest of the cargo on board the said VESSEL now delivered by
me to the Collector of this district and subscribed with my name, contain according to the best of my
knowledge and believe, a full just, and true account of all goods, wares and merchandise now
actually laden on board the said vessel, and of the value thereof; and the foreign places or countries
in which the same are truly intended to be landed. And I further swear that I have mailed to the
Insular Auditor at Manila a true and exact copy of this manifest, and if any other goods, wares or
merchandise shall be laden or put on board the forenamed vessels, previous to her sailing from this
port, I will immediately report The same to the said Collector and Insular Auditor. I do also swear
that I verily believe the duties on all the foreign merchandise and the duties and charges on al
domestic merchandise therein specified have been paid, or secured according to law, and that no
port thereof is intended to be landed within the Philippine Islands, and that if, by distress or other
unavoidable accident, it shall become necessary to relined the same, I will forth with make a just and
true report thereof to the Collector or Customs of the district wherein such distress or accident may
happen. And I further swear that all cargo conveyed on the forenamed vessel with destination to the
Philippine Islands has been duly discharged and accounted for, and that I have not received and will
not convey any letters or other packages not enclosed in properly stamped envelopes sufficient to
cover postage, except those relating to the cargo of the vessel, and that I have delivered to the
proper foreign port all mails placed on board my vessel before her last clearance from the Philippine
Islands.

OATH OF MASTER OF FOREIGN STEAM VESSEL CLEARING WITHOUT PASSENGERS:

I, CAPT. ISIDRITO U. LAGOS solemnly swear (or affirm) That I am the master of the MV
“AQUASALWADOR” steamer, bound from the port of MANILA, PHILIPPINES to the Port of
_________________________________ with freight (or ballast) only, and the said steamer will no
carry upon the present voyage, from any Philippine port, any cabin, steerage, deck or any other class
passenger, nor any person whatever not regularly entered upon the shipping articles of the steamer.
