AFFIDAVIT OF DENIAL

I, Pepito E. Durugas, Filipino, Single, of legal age, and currently residing at 1543 Thousand Sunny
Subdv. Mabalacat City, Pampanga. after being duly sworn in accordance with the law, do hereby depose
and state:

1. THAT I am a law-abiding citizen of good moral character, that I have no criminal or derogatory
records, and that I have no current criminal or civil case before any Philippine court, tribunal, or
agency.

2. THAT I am not the same person as one Pepito E. Durugas who was charged under NPS
DOCKET NO. XII-64921-SK71-POPO for the crime of Armed Robbery in the province of
Pampanga.

3. THAT my middle name is Juarez and not JUNNY. To prove the same, attached is my
Authenticated Birth Certificate with a certified PSO certificate (Annex “A”) and my picture
(Annex “B”)

4. THAT I am signing this affidavit to swear to the facts stated above and to deny categorically and
explicitly under oath that I am not the same person named in the National Bureau of Investigation
docket/files with the aforementioned derogatory/civil/criminal record/s.;
