I, FLORENCIO CABANTING, of legal age, Filipino citizen and resident of

Barangay Cagayungan, Narvacan, Ilocos Sur, am requesting for a revision of a parcel
of agricultural land identified as Lot No. 7703 CAD 623-D located in Barangay
Cagayungan, Narvacan, Ilocos Sur, originally owned/ previously owned by SULPICIO
CABANTING, as Per Tax Dec. 836-F.
