TO WHOM IT MAY CONCERN:

I hereby allow my daughter SHANE B. URETA to attend the limited face-to-

face classes of Grade 12, Section STEM 2 at Regional Science High School for
Region VI, Old Buswang, Kalibo, Aklan.
I acknowledge that I have been informed of the conduct of this implementation
of the limited/expanded face-to-face classes and the benefit that my son/daughter
will derive from his/her participation in this activity. I fully understand that every
precaution to minimize the risk of COVID19 transmission has been observed by
Regional Science High School for Region VI to ensure my child’s safety and since
his/her participation for this activity is voluntary, I shall not hold the school,
employees and personnel responsible for any untoward incident or unforeseen
circumstances that may happen beyond their control.
I therefore affix herewith my name and signature to confirm my decision to the
aforementioned activity.
