JUDICIAL AFFIDAVIT
I am JOVY A. ANTONIO, 40 years old, Filipino citizen, single,
and a resident of Cabanbanan Sur, Gonzaga, Cagayan.

The person interviewing me is Atty. Ramses S. Peralta, with

office address located at G/F Balay Bisita 2, Smart, Gonzaga, Cagayan.
The questions are asked in Filipino and answered by me in Filipino, a
language which I speak and understand.

I am answering the questions fully aware that I do so under oath

and may be held criminally liable for false testimony or perjury.

Q1: Kilala mo ba sina Denis Antonio at Susan Guillermo,

ang mga partido sa kasong ito?
(Do you know Denis Antonio and Susan Guillermo,
the parties in this case?)
A1: Oo, Sir.
(Yes, Sir.)

Q2: Paano mo sila kakilala?

(How do you know them?)
A2: Kapatid ko po si Denis, sir. Si Susan ay asawa ni
Denis, sir.
(Denis is my brother, sir. Susan is Denis’ wife, sir.)

Q3: Paano mo nakilala si Susan Guillermo?

(How did you meet Susan Guillermo?)
A3: Kasama ko po siya sa Boarding House, sir kung saan
nagkakilala sila ni Denis, sir. Tumira sila sakin nung
nag-aayos ng papel si Denis para sa seaman.
(We lived in the same boarding house where she met
Denis, sir, when Denis was processing his papers for
seaman.)

Q4: May mga pagkakataon ba na naobserbahan mo ang

relasyon nila bilang mag-asawa?
(Did you happen to observe the marital relationship of
the parties?)

A4: Opo, sir. Nagpakasal sila noong 2013. Okay naman po

noong magkasama sila pero nagkaproblema sila
noong nagtrabaho si Denis na seaman at kailangan
siya iwan dahil sasakay si Denis ng barko.
(Yes, Sir. They got married in 2013. Their relationship
was smooth at first, when they were still living
together, however, problems started to appear when
Denis worked as a seaman, and had to leave her.)

Q5: Nasabi mo na nagpakasal sila noong 2013,

pagkatapos ng kasal, nagsama ba sila agad?
(You said that they got married in 2013, from that time
did they cohabit together immediately?)
A5: Opo sir, pero sumakay din po si Denis kinalaunan sir.
(Yes sir, however, Denis had to leave for work as a
seafarer, sir.

Q6: Saan sila tumira?

(Where in particularly did they lived together?)
A6: Sa Tuao, Cagayan, lugar po ni Susan, sir.
(In Tuao, Cagayan, the hometown of Susan, Sir.)

Q7: Ano pa ang alam mo sa relasyon nila?

(What else do you know about their relationship?)
A7: Noon una, masaya naman po sila, sir. Pero
nagkaproblema noong sumakay si Denis sa barko.
Nagkaroon ng ibang ka-chat yung babae. Doon siya
humihingi ng payo, imbes na sa amin.
(It was smooth at first, sir. However, problems started
to emerge when Denis worked as a seafarer. The girl
had a chatmate, to whom she asked advice instead
asking from us.

Q8: Ano pa ang nakita mo sa sitwasyon nila?

(What else did you see about their situation?)
A8: Na-develop yung babae dun sa ka-chat niya, sir. Halos
magpakamatay si Denis nung malaman niya na may
ibang lalake ang asawa niya.
(The girl had a relationship with her chatmate, sir.
Denis almost committed suicide when he learned that
his wife was cheating on him.)

Q9: Ano pa ang mga naobserbahan mo?

(Is there any other thing that you observed?)
A9: Nagkahiwalay po sila, sir. Ngayon civil nalang po sila
kasi may kaniya-kaniyang anak na po sila.
(They got separated, sir. They are only civil now, they
already have their own children.)

Q10: Ano pa ang alam mo na nangyari?

(What else do you know that happened)
A10: Nagkahiwalay na po sila, sir ng matagal. At
nagkaroon na sila ng kanya-kanyang pamilya.
Nagdesisyon si Denis na magfile na ng Annument para
makapagsimula na sila ng kanya kanyang buhay, ng
mas matiwasay.
(They got separated already sir, for so long time. And
they already have their own family. Denis decided to
file Declaration of Nullity of Marriage for them to start
anew, separately, in a more peaceful manner.)

Q11: Nagkwento ba sayo si Denis sa mga detalye ng

relasyon nila?
(Did Denis confide to you every detail of their
relationship?)
A11: Opo, sir. At nakita ko po kung paano sila nagsimula
hanggang sa nakipaghiwalay na siya, dahil sa hindi
na nagagampanan ng asawa niya ang tungkulin bilang
isang mabuting asawa.
(Yes, sir. I’ve witnessed how they started until the
moment Denis called it quits because his husband can
no longer fulfill her duties as a good wife.

Q12: Ano ang sumunod na nangyari?

(What happened next?)
A12: Nakipaghiwalay po si Denis at nagdesisyon na i-legal
ang paghihiwalay nila,sir.
(Denis called it quits and decided to end their
relationship legally, sir.)

Q13: I have no more questions to ask from you. Are you

willing to sign this Judicial Affidavit under oath?
A13: Yes, sir.
