Being officially enrolled in this class, I am aware of the guidelines and ought to follow them:

1. The class arrangement will follow AP 5.3 (Online with onsite midterm and final
exams) which states that classes are conducted online where at least 70% is synchronous
and midterm and final examinations are conducted onsite.

2. At least 12 hours (or 3 days) of being absent will result in dropping from the class.

3. Inform Instructors in case of health issues that may cause students to miss a quiz or
major examinations. Likewise, a hard copy of the Medical Certificate (issued by the
attending Physician in real-time) must be submitted to the instructor upon reporting
back to school.

4. Additional concerns not covered herein may be addressed thru MS Teams or in

person during class time or office hours only.

ACADEMIC INTEGRITY

understand that suspected misconduct on given activities/examinations will be reported to the
appropriate office and if established, will result in disciplinary action in accordance with
University rules, policies, and procedures. I may work with others only to the extent allowed by
the Instructor.
