Recommendations: Student Self-Assessment

To ensure admission to top-ranked universities and other good universities, strong recommendation letters from your
teachers and counselors are one of the major prerequisites. All grade XII students applying to universities abroad are
required to fill out this Self-Assessment Form and submit it to their Primary counselors by August 20th, 2022. Students
who will be applying to US universities on early action/decision should aim to submit it earlier. Your answers to the
questions on this form will assist your teachers in writing your recommendation letters, and submitting this form on time
will ensure that your counselor can get you outstanding recommendation letters in good time, and also tailor your advising
to your specific higher education’s goals.
The more specific details you can give us, the more thorough your college recommendation letters will be. Working
thoughtfully on this form will also help you with the entire college application process, especially in preparing for
interviews and writing admission essays. Expect to spend at least 2 hours on this task, which should result in a document
of three to five pages.

Your Academic Performance/ History


1. Describe the academic accomplishment (major paper, science experiment and/or artistic project) you are most proud
of, and explain why you take pride in it. Describe in detail the preparation you undertook, the particular tasks you
executed and the outcomes you achieved.
2. What kind of learner are you? Provide examples that show how you learn and that demonstrate your academic
strengths. For example, describe academic settings or assignments in which you thrived and demonstrated academic
strengths such as analytical skills, creativity, complex problem solving, communication skills, critical thinking, etc.
3. What interests you academically? Explain with an anecdote. Try to be specific and detailed.

4. Extra-curricular activities: List the activities in which you have participated, the number of years and the amount of
time per week you spent, and what you have accomplished and learned from each activity. Consider literary, community
service, leadership positions, after-school programs, school events and groups, jobs, internships, paid or voluntary;
hobbies; sports; travel; music; art; and drama. Use the table template below to present your list. The activities described
are meant purely as examples to guide you to write your own list comprehensively.

7. List your three most distinguishing or most admirable personal qualities. Explain with examples why you think they
are your best qualities and provide examples of how those qualities manifest themselves.
8. What are your personal passions and interests? How do you pursue these interests, in school or outside school? How
do you plan to pursue them in the coming years?
