General Comments:

for TNJO and JEL

- Proper 100% hook up sequence during use of vertical ladders and movement at heights should be clearly explained to all workers..

(Refresher safety training REQUIRED to all operatives assigned to work at heights during Gen.TBM)
Mr. Brigham J. Bates – (TNJO Project Manager)
- State of cleanliness and material arrangement in Boiler Structure …..Excellent!
Mr. Masashi Koga (BTG Civil Manager)
- TNJO should follow JEL’s housekeeping implementation
Mr. Ryosuke Kusaba - BTG Site Manager (A)
- Workers to be closely monitored on their PPE usage habit per task, i.e. FBH, Eye-protection, Hand-gloves.
Mr. S.S. Chan (JEL Site Manager)
- More education to workers in using helmet “chin-straps” as it should be for their safety.

BTG HSE Section (in agreement by all attendees)

- Start punching the ID Cards on those workers smoking in non-designated areas.
This directive should be addressed to ALL workers during General TBM for advance
