I, MR. Shadab Alam Fakhre Alam Asfiya Begam, Aged 27 years

Indian Inhabitant, Residing at Faiz app Room number 602, Sanjay Nagar
Mumbra Thane, do hereby state and declare as under :

I say that I have completed Third Year of LLB in the College known
as M.S. College of Law, Mumbra, situated at Mumbra , Dist. Thane.

I say that while enrolling my name in the said College my name

inadvertently entered/recorded as Alam Shadab Fakhre Asfiya Begam,
instead of my correct/actual name i.e. Shadab Alam Fakhre Alam Asfiya
Begam.

I say that my real name is Shadab Alam Fakhre Alam Asfiya

Begam, which I am desirous to be entered in my College record replacing
my wrongly entered name i.e. Alam Shadab Fakhre Asfiya Begam.

Whatever stated hereinabove is true and correct to the best of my

knowledge and belief and truly affirmed by me without being in any
force, pressure or coercion.
