Guidelines of the accreditation of Service Provider

I. Registration Processing
1. All service provider shall submit and maintain a current and updated file of the following
documents for
- For individual Practitioner
a.) Registration Certificate (Issued by Organization)
b.) Professional License (PRC)
c.) List of services offered including schedule of professional fees
II. For Sole Proprietorship
a.) Registration certificate (DTI)
b.) Mayor’s/Business Permit or its equivalent
c.) List of Services offered including schedule of professional fees
III. For corporation/partnership
a.) Registration certificate (SEC)
b.) Mayor’s/Business Permit or its equivalent
c.) List of Services offered including schedule of professional fees
2. Documents are submitted together with the Letter of Intent address to ADPRO at
_______________from_____________with a non-refundable processing fee of 1000.00
pesos.
3. Processing time shall be within seven (7) working days your receipt of the Letter of
Intent and Eligibility Documents.
4. ADPRO shall issue certificate of registration/accreditation upon assessment that the said
service provider applied with the eligibility requirements. Such certificate shall be valid
within two (2) years from the issuance of the certification.
IV. Rules and responsibilities of ADPRO
1. Processing request for accreditation
2. Provide administrative support in managing contracts, referrals, clients, and other
related functions.
3. Maintain datable and directing of the service providers information/transaction.
4. Ensure that the relevant information of the service providers is isolated in the Online
Portal/websites of the ADPRO and share Social Media Platforms such as FB, IG, Tiktok,
Youtube, etc.
V. Rules and responsibilities of Accredited service provider
1. Maintain a current and updated file of administrative documents and other relevant
referral of the business.
2. Provide services on accordance with the standard professional practitioner and mutual
agreement with the client.
3. Address/assist concerns caused by the clients pertaining for the services offered by the
service provider.
4. Settle accounts due to ADPRO.
