AFFIDAVIT OF DISCREPANCY

 I, ARJAY M. BANAG, of legal age, Filipino, single, and a resident of # 97 Purok 3, Maungib, Pura,
Tarlac, after having been duly sworn to in accordance with law, depose and state THAT:

1. I was born on 21st day of the December 1992 to spouses Marlyn B. Mamaoag and Arsenio L. Banag;

2. I have been using the name Arjay Mamauag Banag in all my documents and transactions;

3. However, when I obtained a copy of my birth certificate, I discovered that my middle name appearing
thereon was erroneously written as "Mamaoag" ;

4. The names “ARJAY Mamauag BANAG” and “ARJAY Mamaoag BANAG” refers to one and the
same person;

5. I execute this Affidavit of Discrepancy of Middle name to attest to the truth of the foregoing facts and
to explain the discrepancy in my birth certificate with regards my surname.

IN WITNESS WHEREOF, I have hereunto set my hand this 1st day of December 2017 in the Province
