DISCHARGE PLAN

Treatment

● Continue with follow up medical care for it is very important to monitor your condition

● Continue maintenance medication as prescribed upon discharge

Medication

Take your Medicines as ordered (be sure to get them from the drug store as you leave the

hospital):

1) Amlodipine 10 mg 1 tab PO to be taken once a day.

2.) Losartan 100 mg 1 tab PO to be taken once a day.

Health Teaching

● Client is advised to bedrest at home/limit activity for 4-6 hours after discharge

● Avoid traumatizing the liver by abdominal palpation

● Regularly take antihypertensive medication to control blood pressure

● Eat a healthy, balanced diet

● Avoid using tobacco products

● Don’t drink alcohol or and drugs not prescribe to you

● Ask your doctor before taking medications not prescribe to you such as over-the-counter

drugs

● Advice the client to expose herself to the sun to serve as an alternate phototherapy to

break down the build up of bilirubin

● Refer for blood transfusion

● Eat folate rich foods such as dark green leafy vegetables

Call your doctor if:

● Your blood pressure is greater than

● You have any new or unusual symptoms

● You have a severe headache

● You have blurry vision

● You have experienced frequent difficulty of breathing

● You have any concerns
