KNOW TO ALL MEN BY THESE PRESENTS:

I, ARLENE DUWANG ANO, Filipino Citizen, of legal age, single, born on the 7th of December,
1982, a resident of Barangay Lamhako Tboli, South Cotabato, and currently working abroad particularly
in the country of Russia as an OFW.

I HEREBY DECLARE, that in the case of an unfortunate event that may happen and may lead to my
passing away;

The condominium unit located at Meridian COHO Floor 3F Unit 0013 of The Meridian – Bluebell Building
located at Brgy. Mambog I, Bacoor City, Cavite, containing an area of Forty-Two (42) Square Meters
more or less. Presently covered by Condominium Certificate of Title (CCT) No. 167-2020000253 shall be
benefited to JERRY MANGABAT CARLOS and will have full rights to the property as part of our contract
and agreement.

None of my Relatives and/or Family Member shall interfere with the above-mentioned property.
