1.0 Introduction

PDO, OXY & Daleel Units Emergency Response Procedure serve to demonstrate that
emergency response arrangements have been systematically assessed and that suitable plans
are in place to respond to all foreseeable emergency situations. It is structured according to the
following elements of emergency response:

• Emergency Response Management;


• Command and Communication;
• Training / Drills for Emergencies;
• Details of Evacuation and Escape Equipment and
• Means of Recovery to a Place of Safety.

In essence, the Emergency Response arrangements used on Alshawamikh Business Units


meet the company’s QHSE Policy. The determination of risk scenarios in the emergency
response procedure is based on the Operations/Function Unit Risk Registers created for all our
operations, whereby the most hazardous risks (major accident hazards) and potential for
escalation into emergency scenarios are paid particular attention to. Apart from the
operations/technical risk registers, Alshawamikh also has a risk register related to Health (HRA)
which is also assessed for potential emergencies from a more health impact driven perspective.

2.0 Emergency Response Management

Alshawamikh has developed an emergency response and management system for responding to
emergencies that potentially may occur at the field and head office. Alshawamikh maintains and
regularly tests the Emergency Response & Management Plan. Key Alshawamikh workers participate
in emergency management training and emergency response simulation exercises.

Emergency planning responses are specific to each work site:


• Hoists/Workover, Wellhead maintenance operations emergency planning
• Pipeline Maintenance operations
• Drill Water Services operations
• Internal planning operations for each base
• Inter-company mutual assistance

This section covers Emergency Response & Management philosophy, analysis and planning for all
Alshawamikh Business Units operations. In AOS we don’t have a medic or clinic but in case of a
medical emergency above first aid we should contact nearby rigs that have medics.

2.1 Emergency Response Management

To achieve Alshawamikh Emergency Response and Management objectives Alshawamikh develops


site specific Emergency Response Plans and carries out regular emergency drills to ensure that the
operating sites and other business operations emergency systems are adequate in terms of reducing
the safety risks and major hazards posed to crew members to ALARP.

Alshawamikh also follows the PDO Emergency Response Procedures PR-1287 & PR 1243.

2.2. Medical Emergency Management

2.2.1 Policy and Strategy

2.2.1.1 Policy Requirements

AlShawamikh Management’s Commitment and QHSE Policy states that “We are all committed to
pursue the goal of no harm to people”.

The Integrated Management System requires us to have emergency response procedures in place,
including plans for medical emergencies and in Alshawamikh we have the PR-IMS-005-Emergency
Response Procedure for Operations.

The group minimum health management standard require exploration & production companies to
plan for managing medical emergencies in compliance with group yellow guide medical emergency
guidelines for management. Where illness or injury occurs at an installation under operational control
(e.g. an office, terminal, a rig or gathering station, production site, an interior or coastal facility etc)
this objective can be achieved by a tiered response (Tier 0 – Tier 4) and this is summarized in table
2.2.1.4.1 below.

2.2.1.2 Medical Emergency Response (MER) Strategy

This procedure adopts a risk-based approach for MER embracing two key concepts: it applies a time
dependent tiered set of responses to an incident, and it sets differing, planned response requirements
depending on the nature of the hazards and the activities on the site.

2.2.1.3 MER Requirements

MER shall be planned, resourced, implemented and documented pursuant to the requirements of to
be able to reasonably meet the response times in Table 2.2.1.4.1, recognizing that the actual MER
response times to an incident depend on various factors, such as time of notification, the incident
location and circumstances (e.g. safety of entering or remaining at incident scene).

2.2.1.4 MER for Third Parties/Contractors

PDO, OXY and Daleel MER teams are always willing to assist contractors such as Alshawamikh in
medical emergencies within their working territories and the MER procedure is the same as the one
outlined in Section 2.

The organization of the tiered Medical Emergency Response Plan for the whole PDO operational
control is detailed in 2.2.1.4.1 table below

Table 2.2.1.4.1 Tier Structure and Response Times

*Note: The four-hour response time in table 2.2.1.4.1 is based on the maximum time that vital life functions
(e.g. respiration, lung function, and blood pressure) can be maintained through artificial respiration, bleeding
control etc. without potentially escalating the casualty’s condition. It is clear, however, that in an actual case
medical judgment should be exercised to readjust the time requirement.

2.3 Emergency Response Philosophy

It is the philosophy of Alshawamikh to put the safety of personnel above the protection of facilities
and continuity of drilling operations, in all emergency situations. In-line with this philosophy, safety
priorities of Alshawamikh are to:

Personnel involved in dealing with the emergency shall follow these priorities when making decisions
and formulating an action plan.

At no time should any action be taken that could endanger the lives of personnel.

HSE Management objectives with regards to Emergency response philosophy is to ensure prompt
and correct response of Alshawamikh staff to each type of emergency in order to minimize harm and
loss to personnel, environment and company assets and to resume normal operations as soon as
possible.

2.4 Emergency Response Analysis

Emergency response is an integral part of the hazard identification and risk assessment process that
is employed in Alshawamikh units operations. Analysis necessarily includes inputs from Rig / Work
Site personnel who have the necessary expert knowledge of the systems and procedures. This
ensures that all hazards identified, have appropriate corresponding emergency response
arrangements. However, formal Escape, Evacuation and Rescue Analysis (EERA) is not done as
part of the Hazards & effects risk assessment because the Rig / Work Site is an open space with no
restrictions on escape and evacuation. Rather, reliance is put on drills and emergency simulation
exercises (see Section 4.2) and the Emergency Response Plan (see Section 2.4 below) to ensure
that emergency response arrangements are adequate.

2.5 Emergency Response Plan

Alshawamikh have corporate and Operations (rig-specific) Emergency Response Plans (ERPs),
which complement Client ERPs (e.g. the PDO ERP Procedures PR-1287 & PR 1243).

The Alshawamikh Units ERP PR & flowchart, see appendix#2 includes:

Alshawamikh also have corporate guidelines and recommended procedures to deal with
emergencies affecting Alshawamikh operations, as documented in Alshawamikh Emergency
Response Plan. This ERP also defines the responsibilities and required actions of key personnel
involved in the operations, as well as the responsibilities and procedures for the Emergency
Response Team (ERT), referring to the AOS ERT for each operation units.

3.0 Command and Communication


3.1 Command during Emergencies

Rig or Site Manager of the Alshawamikh Units is ultimately responsible for safety, health and welfare
of all personnel of the Rig/ Site. As such he has command of the rig/Site and drill site during an
emergency. In spite of this, he will take advice from the Company Man (HSV) or Client Site
Representative as appropriate.

A. On-Site Response
• PDO Drilling Supervisor/ CSR is on-scene Commander (OSC)
• Alshawamikh Rig or Site Manager is Emergency Controller (EC)
• HSE Administrator or HSEA is Scribe & Muster Controller is Night Tool pusher or Driller
• Fire Team leader (Assistant Driller) or fire warden controls fire team
• Rig Medic or First Aider or HSEA controls medical response

B. PDO’s Emergency Control Centers


• Local Emergency Control Centre (LECC) at Interior Area
• Corporate Emergency Co-ordination Centre (CECC) at Mina Al Fahal

C. On-Scene Commander

PDO Drilling Supervisor/ CSR is the On-Scene Commander who is overall in-charge of the
emergency response operations on site, manages the emergency response process and liaises with
LEBC. (Local Emergency Base Controller)

The On Scene Commander will report directly to LEBC and systematically keep them updated with
developments.

Note: If OSC unavailable the Emergency Controller “RIG OR SITE MANAGER” will take on the role.

Below are the generic responsibilities for the OSC, these should be applied to all emergencies in
conjunction with the Emergency specific checklists:

• Leading and managing the emergency response operations


• Assessing emergency situation
• Preventing the escalation of emergency by implementing adequate measures such as
ensuring stabilization or shut-down of well
• Reporting to and liaising with LEBC, providing the following information:
✓ On-site situation updates (Emergency escalating/not escalating)
✓ Personnel situation
✓ Services and support request
• Acting on further instructions and/or suggestions of the LEBC
• Coordinating with EC in the control of emergency teams, activities on site and confirmation
of well/process/utility shutdowns
• Providing information to all personnel of current status and route of action.
• Accounting for all personnel
• Ensuring the safety of all personnel
• Managing of stress in all personnel and ensuring that all personnel remain calm
• Ensuring the protection of the environment

Constant gathering of information from personal observation, EC, Command and Control Board,
other team members and continuously reassessing emergency response and shall Keep instructions
brief and to the point

It is essential that all personnel remain calm. It is the role of the OSC and EC to actively manage
stress and potential panic, both in themselves and others.

D. Emergency Controller

The Emergency Controller is the person who controls the activities of all emergency teams on-site
and confirms the well/process/utility shutdown.

He shall also ensure availability of adequate equipment required in Emergency Response. He shall
ensure that the requirements of this Response Plan are clearly understood by all the field staff and
necessary resources are identified, provided, and maintained at all times.

He reports and liaises with the OSC, working closely as a team, both informing and requesting
external support/assistance as required. His generic responsibilities are

• Proceed to Command & Control Point; Register as "Mustered"


• Put on the EC's Jacket
• Receive briefing on situation including i) What has happened; ii) Current well status iii)
Personnel known/suspected to be involved; iv) Notifications made
• Advises Control Room for sounding of siren and Public announcement
• Ensure the safety of the fire team is considered during all planning stages of the incident
response.
• Mobilize Rig Medic or First Aider or HSEA if not on site
• Organize Fire Team and delegate actions as specified by OSC (process / well shutdowns,
initial firefighting, search and rescue)
• Organize medical team / first aiders and delegate actions as specified by OSC
• Organize & coordinate with Scribe and Muster Controller for personnel injuries / logistic and
other supports.
• On arrival - advise PDO Fire Team of facility status (missing personnel, H2S, potential for
escalation)
• On arrival – advise PDO / PAC medical team on injury status, number of missing
• On arrival – advise earthmoving equipment, cranes etc. of actions as authorized by the OSC
• On arrival – advise ROP requirements to limit access, block roads, inform public
• Regularly update the situation to the relevant operations Manager
• Ensure the proper reporting to the Emergency Duty Manager at Head Office.
• Ensure proper reporting to local government authorities if required in consultation with
Emergency Response Team at Head Office
• Preserve all evidence for subsequent investigation.
• Ensure that investigation of the incident and accident are completed timely.

The above generic responsibilities should be applied to all emergencies.


• Emergency Controller is also responsible for coordination of:
✓ Process shutdown and isolations (Confirm process / well shutdown).
✓ Fire teams, medical, security and resources

E. Scribe

The Scribe is HSE Administrator or HSEA. He is in-charge of keeping a log of events and
information. He reports to and liaises with the EC or OSC.

• Logging events and information on the Command-and-Control Whiteboard / Sheet


including the following records:

✓ Personnel head counts including those in emergency teams

✓ Lists of injured personnel, the type of injury, their current location and destination
• Assist EC to confirm “Shutdown/Equipment/Alert Checklist” items with relevant parties
• Assist EC to update plan
• Prompting considerations to the OSC.

F. Muster Controller

Muster Controller is usually the Night Tool pusher or Driller or other person depending on the site.
He is in-charge of accounting for personnel. He reports to and liaises with the EC or OSC.

The Key Responsibilities of the Muster Controller include:


• Collection of personnel status & information from all parties to identify:

✓ The number of personnel missing and their possible location

✓ The number of personnel injured and the help required


• Relaying information/instructions between the Command-and-Control Post and Muster
Point
• Liaises with medic providing information about injures.

G. The Fire Team Leader

The Fire Team Leader and Fire Team will be made up of personnel normally present at the facility
location. They must have basic firefighting training and valid SCBA certification. The Fire Team
Leader reports to the EC and organizes the Fire Team.

The Key Responsibilities of the Fire Team Leader include:


• Organizes the Fire Team
• Reports to and liaises with the EC on the following information:

✓ Situation Assessment

✓ Possible escalation

✓ Persons rescued

✓ Injured persons found

✓ Trapped personnel

✓ Risk to fire teams

✓ Facility status

✓ Liaises with the medic providing medical status of casualties and medic activities.

H. Medic or First Aider or HSEA

Medic will perform following duties:


• Attend injured personnel
• Assess if injured personnel can be stabilized at camp clinic
• Request additional medical support via OSC or EC.
• Inform OSC/EC if aerial (by flight) medivac will be required
• Prepare injured for travel if required
• Liaise with PDO/PAC medical support on arrival

3.2 Action Checklists for Onsite Response Team

The duties to be performed by each individual with an emergency response role are clearly laid out in
action check list format. In exercises and real emergency situations, these will serve as a memory
aid to ensure that all duties are performed. These checklists are generic and reference must be made
to specific scenarios emergency response in PDO PR-1243 & PR 1287

A. On Scene Commander

The on-Scene commander is responsible for:


• Assisting and advising the EC to ensure the emergency teams are not put at risk in the
implementation of the emergency actions as instructed by the LEBC
• Reporting in situation and support requirements to the OSC Liaison in the LECC

On Scene Commander

4. Go to the emergency scene and gather all possible information on the situation.
Evaluate the event.

5. Proceed to Command-and-Control Point, register as “Mustered”

6. Put on the OSC’s Jacket

7. Ensure Command and Control point is identified (sign posted)

8. Establish and maintain communications with the ETO (Emergency Telephone


Operator)

9. Establish personnel situation with help from Muster Controller. For unaccounted
personnel, consider implementing Search and Rescue IF SAFE TO DO SO
(Approval from LEBC is required)

10. Are fire emergency services required? If so, request PDO Fire emergency
services via ETO

11. Are medical services required? If so, request PDO/PAC medical services via
ETO

12. Organize and brief the Onsite Response Team (Emergency Controller, Scribe,
Muster Controller, Fire Team and Leader, Medic Team and Leader)

13. Start events log

14. Establish and maintain communications with the LEBC (or OSC Liaison)

15. Carry out emergency response procedure for specific emergencies (IF
APPLICABLE)

16. Ensure emergency services are not put at risk in the implementation of the
emergency operations as instructed by the LEBC

17. Act on further instructions from the LEBC

18. Carry out ongoing assessment of the situation and agree “Plan of Action” with
LECC. Continuous accounting of personnel with help from Muster Controller.

For unaccounted personnel consider implementing search and rescue, IF SAFE

3.3 External Emergency Response Support

All Emergencies are normally handled through PDO’s Emergency Control Centres:
• Local Emergency Control Centre (LECC) at Interior Area
• Corporate Emergency Co-ordination Centre (CECC) at Mina Al Fahal

Call outs for local PDO Fire Services & Local PDO Medical Services (including MEDIVAC) is done
by ETO/CCRO if requested by OSC.

PDO Well Engineering Task Force (WETF) performs technical support role to LECC and CECC

The decision to mobilize requirement of resources outside of PDO Emergency Services are taken by
PDO’s Corporate Emergency Co-ordination Team at Mina Al Fahal.

3.3.1 Alshawamikh Management Support

All Alshawamikh employees and senior management shall comply with the requirements of PDO,
OXY & Daleel emergency response policy/procedure.

This emergency response organization will be mobilized at Alshawamikh Head Office if an


emergency arises which cannot be overcome by the staff at the operational sites as well as working
in parallel with PDO LECC & CECC.

3.3.2 Alshawamikh Management Team Responsibilities

Emergency Levels:

• Level -1 emergency will be led by CEO. He will responsible to call out emergency response
team and liaison with client, regularity authority, media and field emergency response team.

• Level-2 emergency will be led by Vice President, Risk & Compliance.

• Level-3 emergency will be led by respective contract holder manager

• The Emergency duty Manager will activate the Boardroom as the Emergency Co-ordination
Room (ECR) in the Head Office. All personnel assigned within the emergency response
organization will immediately get together at the “Emergency Co-ordination Room (ECR)”.
• If first to arrive; check e-mail/ faxed site status report on incident commence the timed event log
(on the whiteboard).
• Enter name and arrival time on whiteboard.
• Keep GSM and phone lines clear and minimize talking time to accept likely incoming calls.
• Participate in team briefing and agree immediate and ongoing actions with emergency.
• Manage and take complete control of the emergency and keep in constant communication with
the site staff.
• Obtain information regarding the nature of the emergency by surveying whiteboard:
✓ Receiving briefing from emergency duty manager

of the emergency response organization, identify appropriate personnel

3.3.3 Alshawamikh Emergency duty Manager

The emergency duty manager will be one of Alshawamikh Managers who is assigned to take the role
of Emergency focal point between Rig / Site work Management and Alshawamikh management team.

• Act as the focal point for emergency response


• Liaison with relevant Government Agencies, Police and Media.
• Immediately establish and maintain contact with the Site Manager.
• Contact and assemble emergency organization at the “Emergency Co-ordination Room (ECR)”
• Logs all ‘confirmed’ information on the information board in the “Emergency Co-ordination Room
(ECR)”
• Provide all details to GM Operations who approves the transfer of incident information to the
‘client’.
• Participate in the team briefing,
• Confirm that emergency services/support have been requested and arrived.
• Brief the arriving emergency team on operational requirements; i.e. what is happening;
requirements; personnel status and possible escalation.
• If applicable-confirm appropriate unit shutdowns have been implemented.

• Decide in consultation with the emergency response organization on the need for an emergency
support team.
• Emergency organization can at this point nominate a more appropriate emergency duty manager
for the particular situation. Alshawamikh emergency number must be reallocated to the new
Emergency Duty Manager GSM.

Ongoing actions:
• Relaying advice and information to and from the site.
• Ensure the operational requirements of the site are communicated, understood and captioned
by the other members of the emergency organization team.

3.4 Communications

This section demonstrates that there are sufficient and effective communications available between
all responsible parties on the Rig/ site in an emergency situation.

• Communication system available at well site comprise of telephone link, HF and VHF radio and
Thuraya Satellite device, Public address system & Emergency Siren.

• Telephone and internet communication system is main communication system and radio installed
in Rig/ site Manager office and vehicle is being used in case of Camp/rig move or unpredicted
communication problems

• Communication in case of emergence are described in HSE manual/Operation manual trough


numbers displayed in Rig/ Site Manager office

• The communication systems arranged on units comprise of Intercoms, 2-way handheld radios
(Walkie Talkies), External Telephone links and, HF, VHF radios, An emergency response room
is established at the Alshawamikh offices (Board Room) for incident management. The total
communication system is highlighted in Alshawamikh Emergency response plan which describes
the process of communication and Organisation role and responsibilities.

4. Training for Emergencies

4.1 Emergency Response Training

The Recruitment and Selection process ensures personnel with appropriate experience; knowledge
competence and skills are appointed. This is so that all personnel fulfil their emergency roles.

made as realistic as possible without disrupting operations. Every emergency drill is followed with a
simulated evacuation drill.

Stretcher Drill
Notify Radio Operator of exact time of drill. Drill to be conducted once every four months, once per crew.

1. Position the dummy in a place where it is likely that a casualty can be found. (e.g. pumps, rig floor,
substructure, tanks etc.).
2. Inform all crew about the drill.
3. Fetch the stretcher and prepare it for use in the vicinity of the (dummy) casualty.
4. Immobilise any broken limbs (if present or simulated) and move the (dummy) casualty onto the
stretcher. Strap him so as to disable all sudden movement.
5. Evacuate him the safest way. Routes and methods of transportation should be varied on
subsequent drills (e.g. training on stairs, man riding winch, crane, loading into bus and car etc.).
6. End drill. Store the dummy & stretcher at their design

Unit X management to send a note to a senior/ well engineer (or equivalent from contractor side) from the

conducted according to the contract requirements and all are reflected in the ILTIZAM assurance plan.

Emergency Drills – Lessons Learning and Tracking

The most credible scenarios and emergency response drills to cover response/preparedness to all
foreseeable events that risk serious injury to the employees including drills tracking sheet are identified
in the ILTIZAM plan as shown in the snapshot below and tracked by AEGIS software on monthly basis.

4.2.1 Incident Debriefing

The Drills reports/ resulting action items are tracked online (AEGIS Action Tracking Register) and updated
on a Monthly basis by the Rig Manager / Site Manager.

Status reports of outstanding action items are issued to the relevant Operations Manager, QHSE
Manager/Director and the Action Party.

When action items cannot be closed out at the initial level by the nominated Action Party, it will be moved
up the line to the next level of authority to another nominated Action Party with a revised target date, until a
satisfactory solution is determined.

All incidents will be reported as per the PDO Incident Reporting procedures and AOS PR-IMS-007 Incident
Reporting & Investigation Procedure. Key learnings from incident investigations, 5-Whys & ICAM
investigations, audit findings, and other performance data analysis will be distributed as applicable using
the existing methods such as TBTs, Bulletins, Monthly QHSE Mailers, HSE Flash Alerts, Safety Stand
downs/ Work stoppage, Saturday’s Gate Meetings, Super Wednesdays, QHSE Culture Building Weekly
Program and HSE Meetings.

Alerts and bulletins will be issued, in a timely manner, to promote organizational learning and foster an
internal ability to improve our performance. For more details refer to the AOS PR-IMS-043 LFI procedure
on the learning from incidents & debriefing.

4.2.2 Performance Monitoring & Continual Improvements

Performance management within the company is focused on setting functional-level goals, monitoring the
methods used to achieve those goals, and then creating ways for management to more effectively achieve
those goals together with their worksite and office-based teams.

By collecting and analyzing specific data and information related to our overall operations and HSE KPIs,
management can determine what effects changes or processes have on performance and then alter those
to help create a more effective system. Performance monitoring and business performance management
is frequently used to continually improve the performance of personnel and management within all functions.

The learning from Emergency Response drills are tracked and implemented by AEGIS to close identified
gaps as part of a continuous improvement process. AEGIS is used to capture data and dashboards are
available via AEGIS to provide an overview of performance over time.

In addition AEGIS monitors the ILTIZAM performance, conduction the audit/inspections, raising NCR’s &
closing out the action items by sending notifications to the assigned action party.

4.3 HSE Inductions

All personnel arriving on Alshawamikh Units are inducted into emergency procedures and alarms.

5. Details of for Evacuation and escape Equipment


5.1 Evacuation and Escape Systems

Evacuation and Escape arrangements with reference to emergency plans are described as below:

5.1.1 H2 S

Evacuation and Escape arrangements for H2S response is reproduced below as an example:
• On hearing of emergency alarm the driller/ HSEA shall activate the air horn & don the escape set
together with his crew.
• Well site detection systems indicate low level H2S from 5 ppm and sound alarm at 10 ppm.
Personal H2S Detectors will only sound at +/- 10 ppm.
• The driller/ HSEA shall secure the well with the assistance of Drill/ site crew as per standard well
shut- in procedure.
• If the Derrick man is up on the monkey board, then he shall don the escape set, climb down the
derrick & join the drill crew if required.
• After the well is safely secured, the drill/ site crew shall proceed in an upwind or crosswind
direction to the upwind H2S cabin.
• The driller/ HSEA shall note H2S reading from the H2S monitoring unit before moving to the
upwind H2S cabin with the crew.
• Mud tester shall don escape set, secure the worksite & move to the upwind H2S cabin. Any other
staff present shall don escape set move to muster point without delay on hearing the alarm.
• All other personal on the rig site shall proceed to the upwind H2S cabin, dedicated escape set
wears after having donned their escape set & secured their work place.
• The gate shall be locked by the Gate guard & the control board shall be taken to the secondary
assembly point if the wind direction requires the crew to assembly there.
• The Emergency controller shall use the H2S card board to make a head count of the personnel
at the assembly point

Search for missing person:

If any person is found missing then the following actions shall be taken:
• As immediate response two search team A & B & a backup team shall be organized. Each team
shall consist of 2 persons who shall wear 45 minutes breathing apparatus. The senior member
of a search team shall be the search team leader of that particular team. He shall be equipped
with the respective search route plan & conduct the search according to plan.
• If any of rig personnel is sleeping in rig accommodation not being aware of the situation, he shall
be rescued by the search team & brought to upwind assembly point after he has donned his
escape set

• If H2S victim is located, the search team will immediately move the victim to the safe assembly
point for first aid & immediate action will be taken by Medic to review casualty with 100% Oxygen
treatment using Oxygen Resuscitator, then casualty shall be rushed to the nearest clinic for
expert medical care.
• If no victim is found, the search team shall return to the assembly point and wait further instruction.
• The backup team will remain on stand by the assembly point for providing any assistance to the
2 search teams if required.
• The Emergency controller at the assembly Point shall designate 2 reliable crew members to don
45-minute B.A. sets & send them to survey the location with portable H2S detectors. On
completion of survey they will return to the assembly point & report to the Emergency controller.
• The Emergency controller will discuss the findings with PDO, OXY & Daleel representative on
site (OSC) & inform all parties of the situation, including local emergency base center (Tel: PDO
24385555 – OXY 25693333 – Daleel 80074400 ) & wait for further instructions.
• The OSC shall assess the situation and assign duties to each person to bring the situation under
control. All persons remaining on the site shall use the “buddy system”. If the gas is ignited, the
H2S will be converted to Sulphur dioxide which is also poisonous. Therefore, it shall not be
assumed that area is safe after the gas is ignited. Continue to observe emergency procedures
and follow instructions of the OSC.
• All ignition sources must be shut down IF SAFE TO DO SO. This may include making the well
safe and shut down machinery or engines in the affected area.

Potential ignition sources include the following:


• Smokers’ materials e.g. cigarettes and matches
• Naked Flames
• Electrical, gas or oil-fired heaters (fixed or portable)
• Hot processes e.g. welding or grinding work
• Engines or boilers including car engines
• Machinery
• Faulty or misused electrical equipment
• Lighting Equipment e.g. Halogen Lamps
• Hot surfaces and obstruction of equipment ventilation e.g. office equipment
• Friction e.g. from loose bearings or drive belts
• Static electricity
• GSM / Mobile phones
• Only after “All clear” is given by LEBC work is to be resumed as normal.

First Aid Treatment of H2S Casualties

Inhalation
1. Before attempting a rescue, don your SCBA. You too, can become a victim.
2. If the victim is unconscious and not breathing take him, at once to the “Safe Briefing

Area” and use the resuscitator until normal breathing is restored.

3. Symptoms may pass rapidly; however, keep the patient warm.

4. Get patient to nearest clinic as soon as possible.

Contact with Skin

Skin absorption is very low. Skin discoloration is possible after contact with liquids
containing Hydrogen Sulphide. If such contact is suspected, the area shall be thoroughly
washed.

Contact with Eyes

Eye contact with liquid and/or gas containing Hydrogen Sulphide will cause painful irritation
(conjunctivitis). Keep patient in a darkened room, apply ice on forehead, and send to hospital / clinic.
Eye irritation caused by exposure to Hydrogen Sulphide requires treatment by a physician preferably
an eye specialist. The progress to recovery in these cases is usually good.

First Aid and Medical Advice

H2S poisoning is a life-threatening condition characterized by loss of consciousness and associated


respiratory failure. Treatment must therefore be rapid to be effective.

Remove the affected person to an H2S free area.

If breathing has stopped apply artificial respiration and continue until a mechanical resuscitator is
available to support respiration or until the victim resumes breathing.

Obtain medical attention immediately.

If eye contamination is suspected flush the eyes with clean water for up to 10 minutes.

Subsequent to the resuscitation procedures described, keep the victim under observation and treat
symptomatically as indicated by the patient’s condition.

If the victim is moved to a medical facility, ensure that information on H2S poisoning is supplied to the
medical staff.

Other points

Harmful exposures only occur by inhalation. Intake via other routes is of no medical significance.

The use of 4-dimethyl amino phenol as an antidote for the systemic toxic effects of H 2S is not

Recommended, Experience has shown that, following successful resuscitation, recovery will usually
be rapid and complete without the use of an antidote.

5.1.2 Fire at Rig Site

Primary fire-fighting must not endanger personnel. The first priority in any emergency is the safety of
personnel. Primary fire-fighting may only be carried out by trained personnel using the appropriate
type of fire extinguisher.

When fire is detected in an area, the automatic fire/smoke detection system will most probably sense
the fire/smoke and raise the fire alarm. In certain circumstances, the fire may be too small to have
triggered the automatic alarm. Under these circumstances, the fire will have to be reported by the
person who discovers the fire.

Manual Activation of Alarm

When a fire is detected anywhere on-site, the first person who discovers the fire will
• Shout ‘Fire, Fire, Fire’
• Activate the nearest fire alarm
• Attempt to put out the fire if fire is small and IF SAFE TO DO SO
• Evacuate the area and go directly to the upwind muster point
• Emergency controller must confirm with driller that the well is secured
• Emergency controller must ensure that Medic is on location with proper First aid Equipment
• Emergency controller must immediately coordinate with PDO representative (OSC) and advise
him of the exact location, size and nature of the fire.
• Emergency controller must ensure that no person is missing as per head count.

• Emergency controller must confirm the location of any personnel who may be missing or trapped
in the area and ensure that all personnel are evacuated to muster area by implementing search
& rescue IF SAFE TO DO SO.
• All injured personnel are provided with the required first aid/medical treatment (including
evacuation, as required).
• The Medic or First Aiders or HSEA will render First Aid to all Injured Personnel WHERE IT
IS SAFE TO DO SO. First Aid may range from minor lacerations / bruises to broken limbs,
burns, respiratory problems and major trauma. The Medic or First Aider or HSEA will be
responsible for the medical needs of the injured personnel.
• PDO medical service will be mobilized if additional medical support is required. Critically
injured personnel should be transported by MEDEVAC where practicable. All injured
personnel who require further medical attention shall be transported as soon as
possible to the nearest medical facility.

Assessment of Situation:

During the fire event, the OSC will continuously assess the situation. The OSC will consider the
following:
• Can the onsite Fire Team fight the fire or is PDO Fire Service required?
• Are any personnel injured and is PDO medical service required?
• Is Search and Rescue required?
• Is ROP assistance required?
• Check for the presence of explosives in the area.

Control Measures:
• Depending upon the fire event, Isolating services from the fire e.g. fuel, gas, electricity and turn
off any fans or blowers
• OSC shall inform surrounding facilities or communities that are affected by the smoke or where
there is a potential of the fire event escalating.
• OSC will ensure clear access route to fire and control the access
• Ensure to post person at access road entrance to guide emergency services
• Ensure not to enter area in case of a fire near explosives
• The OSC maintains communications with the LEBC and act on instructions provided by the LEBC.
• The OSC also maintains communication with muster stations, emergency teams via the Public
Address. (PA) system.
• The OSC will ensure that the EC is in communication with fire teams at all times.

The OSC shall carry out ongoing evaluations throughout the fire event, considering the following:
• Is the fire escalating or under control?
• Is ‘Search and Rescue’ operation required? (with approval from the LEBC)
• Is MEDEVAC required?

5.1.3 Fire at Camp Site


When a fire is detected anywhere on-site, the first person who discovers the fire will
• Shout ‘Fire, Fire, Fire’
• Activate the nearest fire alarm
• Attempt to put out the fire if fire is small and IF SAFE TO DO SO
• Evacuate the area and go directly to the upwind muster point
• The Journey Manager to immediately inform Emergency controller who has to go directly to the
camps site
• Emergency controller must ensure the Medic is on location with proper First Aid equipment
• Emergency controller must ensure that Sr. Mechanic, Electrician are at the camp site in case it
is needed to cut the power off the area at which the major fire or explosion occurred or try to cut
the fuel off the area at which the major fire or explosion occurred
• Emergency controller must immediately coordinate with PDO representative (OSC) and advise
him of the exact location, size and nature of the fire.
• Emergency controller must ensure that no person is missing as per head count.
• Emergency controller must confirm the location of any personnel who may be missing or trapped
in the area and ensure that all personnel are evacuated to muster area by implementing search
& rescue IF SAFE TO DO SO.
• All injured personnel are provided with the required first aid/medical treatment (including
evacuation, as required).
• If Emergency controller and OSC decide that an evacuation is required, they have to arrange for
transportation to take the crews to a safe location. In that case, communication should be
maintained using the vehicle radio.

5.1.4 Road Traffic Accidents


• If the accident took place near the rig location, then Emergency controller (or his designate) and
the Medic should proceed to the site of accident.
• Medic to ensure he has proper First Aid equipment.
• Emergency controller to immediately contact with OSC and notify him.
• Emergency controller will also notify to Alshawamikh On Duty Manager
• If Emergency controller and OSC decide that an evacuation is required after consulting PDO
medic, they have to arrange for transportation to take the injured person / persons to the nearest
PDO medical facility;
• Injured person / persons shall be evacuated in Alshawamikh ambulance by medic or First Aider
or HSEA to nearest PDO medical facility.
• Ambulance shall be followed by another vehicle as a backup.
• Medic will keep on rendering first aid as & when required on the way to PDO clinic.

• If the accident takes place far from the rig location, then Emergency controller to immediately
contact OSC who will contact LECC.

5.1.5 Rescue from height

If a worker falls and is suspended by a safety harness, implement the emergency response plan by
following the steps below.

Note: It’s important to know your role.

• The Rig Manager or his designate takes control of the situation.


• The Rig Manager sounds the emergency alarm. All workers in the immediate vicinity of the incident stop
working. The site supervisor quickly evaluates the situation and identifies any further hazards that could
arise.
• The site supervisor or their designate goes to get help if workers are close by. If no one is close enough,
the site supervisor calls for help.
• The site supervisor calls 9999 to notify local police, fire, and ambulance if required.
• The crane operator remains on standby. The operator frees the hook and waits for further direction in
case the designated rescue team must perform a basket rescue.
• The site supervisor (or a worker assigned to the task) isolates the accident zone and its perimeter to limit
further exposure.
• The site supervisor (or a worker assigned to the task) moves all non-affected personnel to a safe zone
or directs them to remain where they are.
• The site supervisor enables radio silence on the jobsite, except for crisis communications from
emergency responders. These communications are conducted on a pre-selected "emergency only"
radio channel.
• The site supervisor sends a designated worker to the site gate to meet the response team (police,
medical, fire, etc.) and ensure that they have a safe access path to the accident scene.
• The site supervisor assembles the emergency rescue team at the accident site as quickly as possible to
determine the best rescue procedure for the situation.

Rescue from Height Scenarios


Each working at heights operation shall have a rescue plan covering all expected scenarios. These plans
shall be communicated to all job parties. If a worker needs to be rescued from height (conscious or
unconscious), or if a worker falls and is suspended by a safety harness, implement the emergency
response plan by following the steps below.

5.1.5.1 Rescue from Monkey board platform using R.I.D.E system

In the event the worker is conscious and can`t use the Derrick/Mast ladder to exit the Monkey
board.
The Derrick or Mast will have an auxiliary mean of escape R.I.D.E installed prior to personnel
working in the Derrick. The auxiliary escape R.I.D.E is a convenient and ready mean of escape
from the Monkey board. The R.I.D.E route and landing point will be kept clear of obstructions.

If an emergency occurs:
- The worker in one motion as he walk off the working platform the safety cable will contact the
lower part of your thigh & release the shuttle’s secondary securement
- Step onto the shuttle seat
- The weight activated release mechanism will then disengage the shuttle bringing the worker
down
- The R.I.D.E system brake will automatically control your descent
- The R.I.D.E system is equipped with a redundant brake & is simply operated by pulling down

- Driller to activate rescue plan from Monkey board using working at height rescue kit
- Inform RM
- Mobilize rescue team, Crane, Ambulance
- Rescue team (2 members) prepare stretcher at rig floor level
- Rescue team (2 members) climb to the Monkey board with working at height rescue kit to assist
worker (ensure 100% tie off)
- Rescue team to install working at height rescue kit as per manufacturer recommendation
- If worker is in reach, rescue team to anchor the rescue kit hook to workers safety harness
dorsal D ring
- If worker is out of reach, rescue team to use telescopic pole to anchor the rescue kit hook to

Rescue Procedures

The following rescue procedures are ordered (A) through (D), with (A) being the preferred method and (D)
being the method used when there is no other means of rescue.

A. Elevating Work Platform Rescue


If an elevating work platform (EWP) is available on site and the suspended worker can be reached by the
platform, follow the procedure below.

• Bring the EWP to the accident site and use it to reach the suspended worker.

• Ensure that rescue workers are wearing full-body harnesses attached to appropriate anchors in the
EWP.
• Ensure that the EWP has the load capacity for both the rescuer(s) and the fallen worker. If the
fallen worker is not conscious, two rescuers will probably be needed to safely handle the weight of
the fallen worker.
• Position the EWP platform below the worker and disconnect the worker’s lanyard when it is safe to
do so. When the worker is safely on the EWP, reattach the lanyard to an appropriate anchor point
on the EWP if possible.
• Lower the worker to a safe location and administer first aid. Treat the worker for suspension trauma
and any other injury.
▪ Arrange transportation to hospital if required.

B. Ladder Rescue
If an elevating work platform is not available, use ladders to rescue the fallen worker with the procedure
outlined below.

• If the fallen worker is suspended from a lifeline, move the worker (if possible) to an area that
rescuers can access safely with a ladder.
• Set up the appropriate ladder(s) to reach the fallen worker.
• Rig separate lifelines for rescuers to use while carrying out the rescue from the ladder(s).
• If the fallen worker is not conscious or cannot reliably help with the rescue, at least two rescuers
may be needed.
• If the fallen worker is suspended directly from a lanyard or a lifeline, securely attach a separate
lowering line to the harness.
• Other rescuers on the ground (or closest work surface) should lower the fallen worker while the
rescuer on the ladder guides the fallen worker to the ground (or work surface).
• Once the fallen worker has been brought to a safe location, administer first aid and treat the person
for suspension trauma and any other injury.
• Arrange transportation to hospital if required.

C. Rescue from Work Area or Floor Below


If the fallen worker is suspended near a work area and can be safely reached from the floor below or the
area from which they fell, use the following procedure.

• Ensure that rescuers are protected against falling.


• If possible, securely attach a second line to the fallen worker’s harness to help rescuers pull the
fallen worker to a safe area. You will need at least two strong workers to pull someone up to the
level from which they fell.
• Take up any slack in the retrieving line to avoid slippage.
• Once the worker has been brought to a safe location, administer first aid and treat the person for
suspension trauma and any other injury.
• Arrange transportation to hospital if required.

D. Basket Rescue
If a worker has fallen and is suspended in an inaccessible area, you may need to perform a basket rescue.

For basket rescues, the basket must be designed by a professional engineer in accordance with good
manufacturing processes to withstand all loads to which it may be subjected. It must be kept on site at all
times in an accessible location where it is clear of material or other equipment. Fit the rescue basket with
appropriate rigging for quick hookup by the crane operator.

Always keep the following items in the rescue basket.


First-aid kit
• Three lanyards equipped with shock absorbers
• One full-body harness
• Tag line attached to the basket at all times
• Descent controller rescue device in good working condition
• Secondary safety line to tie the basket above the headache ball of the crane.

To perform a basket rescue, follow the steps below.

▪ Make sure preferred methods A, B, and C are not possible.

• Notify the crane operator right away to position the crane to attach the basket.

• While the basket is being attached, the crew leader checks that all safety rigging is done and all the
required safety equipment is available.

• With two rescuers in the basket, hoist it to a position that is above and as close as possible to the fallen
worker. A designated worker on the ground guides the basket with a tag line. The designated worker
must make sure that when the rescue basket reaches the right elevation, the door of the basket is
facing the structural steel to provide an easy exit for rescuer #1.

• Rescuer #1 exits the rescue basket and gets into a position to reach the fallen worker. When doing this,
rescuer #1 must be tied-off at all times to either the structure or the rescue basket.

• Rescuer #2, who is still in the rescue basket, lowers the line that will be used to retrieve the worker.
Rescuer #2 attaches an extra lanyard to the line if required.
• Rescuer #1 assesses the fallen worker for injuries and then decides how to proceed (i.e., treat injuries
first, guide the fallen worker into the rescue basket, or lower the basket to the ground with the fallen
worker attached to it).

• Once the fallen worker has been brought to a safe location, administer first aid. Treat the person for
suspension trauma and any other injury.
• Arrange transportation to hospital. A designated worker must accompany the injured worker to hospital.

If the basket rescue is the method used, keep the following points in mind.

• Perform a basket rescue only when it is not possible to use conventional equipment to rescue the
fallen worker in a safe manner.
• Never exceed the maximum number of workers in the basket as indicated on the nameplate.
• Ensure that a competent worker inspects the crane and equipment being used prior to lifting
rescuers.

• Always equip the crane with a fail-safe mechanism to prevent the boom from descending in the
event of a power source or system failure.

• Maintain an adequate means of communication between the rescuers in the basket and the crane
operator at all times.
• Ensure that workers in the rescue basket wear full-body safety harnesses attached to a lanyard and
anchored to appropriate points in the basket at all times.
• Make sure that all rigging used to attach the rescue basket to the hook of a load line has a safety
factor of 10 against failure. There should be a safety line attached to the load line directly from the
basket.

• Do not allow cranes to travel while rescuers are in the basket.

• Do not use suspended rescue baskets during high winds, electrical storms, snow, ice, sleet, or
other adverse conditions that could affect the safety of personnel on the platform or in the basket.

Post-Rescue Procedure

All non-affected workers should remain in the designated safe gathering zone until the site supervisor
notifies them to do otherwise.

The site supervisor and health and safety representative should:


• Begin the accident investigation.
• Quarantine all fall-arrest equipment that may have been subjected to fall fatigue effects and/or
shock loading for further investigation.
• Secure the area (the OHSA requires that an accident scene not be disturbed where a fatal or
critical injury has occurred).
• Determine whether or not the jobsite-specific rescue and evacuation plans were followed as
designed.
• Record modifications or additions to the plans that the rescue team deems necessary.
• Record all documented communications with fire, police, PDO, and other contractors involved.
• Record all documented statements from employees, witnesses, and others.
• Save all photographs of the incident.

• Record all key information such as dates, time, weather, general site conditions, and specific
accident locales including sketches of the immediate incident area, complete with measurements if
applicable.

Rescue from Height --- Dial PDO emergency Number 5555

Following procedures shall be adhered in case of an emergency where Rescue from height operation is
necessary:

a. Upon hearing an Alarm every one assembles into appropriate muster point.
b. Rig Manager/ Night Tool Pusher mobilize rescue team consisting two persons out of which one hand
preferably trained in First Aid
c. Rescue team don up full body harness and climb up mast attaching fall arrestors and proceed to
monkey board.
d. In the mean while RM / NTP instruct a person to bring the basket stretcher and then raise the stretcher
to Floor by crane. Ensure that 4-legged webbing bridle is attached with and a tag line. Keep First Aid
Box in the stretcher. Let the crane be stand by near the floor. Ask driller to man the driller console.
e. Driller to ensure that traveling block is brought down to floor level
f. Hook up webbing bridle attached on basket stretcher to Manriding winch /winch line; raise the
stretcher to monkey board, taking support of the drill line to avoid stretcher to swing.
g. Ensure that Rescue teams at monkey board are secured with fall arrestor / lanyard. Get the stretcher
on to monkey board and place it appropriately depending upon IP position.
h. One of the rescue team members ensures that the webbing bridle on one side is removed from the
stretcher and all the fasteners are out. Other person (First Aider) checks IP’s condition, console him
and provide first aid.
i. Both persons lift the IP gently and lay IP in the stretcher. Ensure IP is fastened with safety belt /
fastener. Hook back the webbing bridle.
j. Rescue team leader signal to winch operator/driller and then slowly lift the stretcher and guide into
position for lowering
k. Winch operator brings down the stretcher to rig floor in same manner as explained in (e). Rescue team
member shall guide the stretcher by tag line.
l. As the stretcher reaches to floor, unhook the winch line and then hook up with crane and then lower to
ground where ambulance/ emergency vehicle is stand by.
m. Place the stretcher with IP directly into the emergency vehicle and then drive to nearest PDO
clinic/hospital

Important note: Ensure that a fall arrestor line is attached to the Stretcher while lowering, as a
backup in case of any failure of Man riding winch line.

2. Emergency team:

The crew was gathered in assembly point and informed the emergency situation of injured person at
derrick, and organized a rescuing team as follows:

- RM to lead and signals the crane operator

- AD and a senior floor man/first aider to climb to derrick to rescue the IP.

- A crane operator to operate the crane.

- A Roustabout to hold the tag line.

- One more First Aider at ground.

- Two more helping hands to help in lifting IP.

- Driver to drive the emergency Vehicle.

3. Drill events:

- The basket stretcher was made ready with 4-legged webbing bridles, tag line attached, first Aid box
and belts prior to sending basket to derrick for rescuing IP.

- RM was team leader of the rescue operation from height and signalled the crane operator.

- AD and a First Aider (Sr. floor man) fully equipped with full body harness and fall arrestors climbed
on the derrick to rescue the IP.

- Following signals by team leader, the crane Operator has lifted the basket stretcher up to monkey
board with First Aid box, after driller has brought down the travelling block to the floor level, one crew
member was holding the tag line to prevent swinging of the basket stretcher.

- The stretcher was raised up to monkey board and the two rescuers lowered it on derrick board, gave
necessary first aid to the IP, lifted and laid him on stretcher, engaged the 4-legged webbing bridles
to the crane hook and signalled to the team leader of their task completion.

- Another First Aider who was waiting for The IP to come down to gave him necessary first aid

- Two Roustabouts made emergency vehicle stretcher ready to shift the IP on it upon reaching on
ground.

- The driver was alert by keeping the Emergency vehicle ready with rear doors opened for a quick
shifting IP to Vehicle and then placed the victim in the emergency vehicle

5.1.6 Rescue from Confined Space

In the event of an emergency of any type in the confined space the following shall be followed:
• The attendant or stand by man should immediately report Unit Manager and inform that rescue
is needed from a confided space.
• Unit manager EC to notify OSC (HSV) who in turn notifies Client Emergency Response Team.
• If rescue from within the confined space is required confined space entry rescue plan shall be
developed by unit manager EC and OSC.
• Retrieval and rescue equipment to be arranged for rescue mission.
• EC to select rescuers and arrange site medic and ambulance medic
• The oxygen and H2S level will be tested prior to entering the confined space. The rescue team

shall don BA sets. (No entry will be permitted without BA if the atmosphere reading is outside
acceptable limits).
• All entrants into a confined space shall wear a full body harness continually. The use of a harness
is not required if it will create a greater hazard to the wearer.
• Attendant shall have necessary communications capability to raise an alarm and summon
help.
• A support device shall be provided at the entrance to the confined space, fitted with an anti-
fall inertia reel and a method of personnel retrieval.
• Attendant will immediately ask for help if need arises to rescue Entrant.
• OSC shall coordinate the rescue and interact with Asst. OSC on situation, injuries, and medical
assistance required whenever is needed.
• The casualty shall only be removed from the confined space after examination unless the
environment causes an immediate threat to life. If the oxygen level is less than minimum required,
then consider donning BA on IP.
• Once removed from the area, the rescue team will assist with the transportation of injured parties
to the Clinic or if required, evacuate to nearby Hospital. Follow MERP.
• The following equipment is required to rescue a casualty in each area and shall be brought to
the area if not already there:
- Intrinsically safe torch and handheld radios.
- Two full body harness.
- Rescue line/Tripod.
- Potable gas detector.
- Two BA sets fully charged for rescue team.
- One 10-minute Escape Pack with hood for injured person.
- Stretcher rescue type

Confined Space Scenarios


Each confined space entry operation shall have a rescue plan covering all expected scenarios. These plans
shall be communicated to all job parties. If a worker needs to be rescued from confined spaces (conscious
or unconscious), implement the emergency response plan by following the steps below

5.1.6.1 Rescue from cellar

In the event the worker is whether conscious or unconscious and can’t use the cellar ladder stairs
to exit the cellar
If an emergency occurs:
- Driller to activate rescue plan from cellar
- Inform RM
- Mobilize rescue team, Ambulance
- Rescue team (2 members) prepare stretcher
- Rescue team (2 members) to go inside cellar to assist worker (Perform Gas test). Wear SCBA
if required
- Position MEWP to install chain block on pad eye below rotary table

5.1.6.2 Rescue from Mud tank (ensure lift plan available)

In the event the worker is whether conscious or unconscious and can’t use the Mud tanks ladder to
exit the Mud Tank

- Mobilize rescue team, Ambulance, crane
- Rescue team (2 members) prepare stretcher and recue tripod

5.1.7 Snake Bite


• There are numerous venomous snake species and scorpions found in Oman. Reassuring a bite
casualty is vital, for if the casualty keeps still and calm, the spread of venom may be delayed.
• Make sure there are no risks to you and to go to aid the casualty.
• Reassure the casualty and get him to lie flat.
• Restrict movements of affected area to reduce the flow of venom.
• Treat the patient instead of attempting to kill the snake. It will cause delay in the treatment and
expose others to danger, but try to identify the snake type to be able to use the right anti-venom.
• Contact the nearest hospital or PDO doctor explain the situation and arrange for an ambulance.
• Medic or First Aider or HSEA must render first aid to the victim
• Wash the bitten area thoroughly with anti-septic solution.
• Remove any rings or constricting items because the affected area may swell up.
• Apply a dressing (preferably Crape bandage) by gently wrapping it spirally two to four inches
above the bite. This should not be tight enough to cause obstruction of blood flow.
• Apply a splint to the whole limb and ensure immobilization. Keep the affected limb to lower side
if possible
• Monitor the person's vital signs -- temperature, pulse, rate of breathing, and blood pressure.

• If there are signs of shock (such as paleness), lay the victim flat, raise the feet about a foot, and
cover the victim with a blanket.
• Maintain Intravenous line to the victim & give corticosteroid if time permits
• Watch the IP very carefully for the signs of systemic envenomation i.e. repeated vomiting,
drowsiness, tachycardia, sweating, pallor, low blood pressure.
• Symptoms that indicate paralysis include Generalized weakness, difficulty in breathing, blurred
vision, hyperacusis (high level of sensitivity to sound) headache, dizziness, vertigo and signs of
autonomic hyperactivity.
• Transport the patient to the nearest Hospital for further treatment.
• During transfer, monitor the hand/foot signs for circulatory failure, lack of pulse in the limbs, blue
discoloration etc…. the bandage may have to be loosened in such cases

Anti-snake serum:
• Anti-snake serum has its own range of serious side effects and should only be used by qualified
medical personnel
• Alshawamikh rig Medics are not allowed to use or keep the anti-snake serum on location

5.1.8 Scorpion Bite

In case of a scorpion bite, the action plan is almost the same as of the snakebite. The only differences
are:
• Scorpion bites are more painful locally and an injectable (Intramuscular) painkiller is usually helpful.
• Scorpion bites are capable of causing an anaphylaxis shock more as compared to snake bites.
Pain may be a triggering factor again.
• All symptomatic (patient showing symptoms) patients should be evacuated using all the
precautions listed under snake bites.
• An intramuscular injection of corticosteroid and maintenance of I/V line if possible, can be useful.

5.1.9 Flooding
• The emergency alarm has to be raised, all crew members shall evacuate the area & go to muster
point
• Emergency controller must confirm with driller that the well is secured
• Emergency controller must ensure that Medic or First Aider or HSEA is on location with proper
First Aid equipment
• Emergency controller should try to secure major electrical and mechanical equipment IF SAFE
TO DO SO.
• Emergency controller must immediately coordinate with PDO representative (OSC)
• Emergency controller must ensure that no persons are missing as per the headcount
• Emergency controller must confirm the location of any personnel who may be missing and ensure
that all personnel are evacuated to muster area by implementing search & rescue IF SAFE TO
DO SO.

• In case the missing person cannot be found, OSC should immediately contact PDO emergency
number.
• Emergency controller should inform to Alshawamikh on duty Manager.
• The Medic or HSEA or First Aiders will render First Aid to any injured Personnel WHERE IT IS
SAFE TO DO SO.
• PDO medical service will be mobilized if additional medical support is required
• If Emergency Controller & OSC decide that an evacuation is required, they have to arrange for
transportation to take the crews to a safe location. Communication should be maintained using
Thuraya.
• If this cannot be done due to floods around, then the safest high point must be located and all
crew to be taken there.
• Under no circumstances, flowing wadis or valleys to be crossed.

5.1.10 Man Missing / Lost

For vehicles coming or leaving the site if a vehicle does not arrive at destination within one hour of
the estimated time of arrival (ETA), the situation will be announced as “Man Missing Situation” and
the site manager will initiate a local search for the driver / vehicle. This search will be limited to the
following:
• Gather missing personnel data
• Call the journey manager or location of residence
• The driver’s accommodation if applicable
• Accommodation of any passengers if applicable
• Accommodation of any friends if known
• Camp recreational room
• Local shops
• Local car parks
• Mosque

If the above - mentioned search is unsuccessful, the site manager will announce the situation as
“Man lost Situation”.

At operational site / unit site:

If a person does not appear within one hour, the situation will be announced as “Man Missing
Situation” and Rig or Site Manager does the following:
• Assembles all rig / unit personnel and determines when the person was last seen and what
he was last doing
• Organizes a search in the rig / unit area
• Arranges a local search using all available means
• Ensures that the person has or has not left the rig / unit by checking all transport movement
since his disappearance.

• Announce the situation as “Man Lost Situation” and informs the duty emergency manager
who informs the CEO / Operation or Project Manager / QHSE Manager

Man lost situation


• Unit Manager (EC) will inform Client site representative (OSC) who will inform LEBC.
• EC will ensure all precautions are in place
• EC will organize two search team A & B & a backup team & brief search team. Each team
shall consist of 2 persons. The senior member of a search team shall be the search team
leader of that particular team. He shall be equipped with the respective search route plan &
conduct the search according to plan.
• EC will get authorization from OSC to start search (who in turn get authorization from LEBC)

Action of Search Team Leader:


• The Search Leader shall assemble a team to assist him, comprising of a Base / HSE
Administrator (to log radio calls, events and timings) and a camp boss.
• The Search Leader will prepare a search plan of the area concentrating on the route back
into camp and last known position. A briefing with the search team will be held to include all
those involved in the search before the search commences. Watches should be
synchronized at the briefing.
• If deemed necessary the Search Leader will organize search which will be limited to black
top and graded roads and other known, well defined tracks. The search will concentrate on
making visual or radio contact with the missing person rather than making physical contact.
The only exception to this is if there is definite knowledge of a life-threatening situation.
• The Search Leader will inform medic to standby and will organize the preparation of
ambulance and a back-up vehicle equipped with the appropriate communication equipment.
• The Search Leader will ask EC to provide an up to date area map.
• During the initial search two personnel will man each vehicle. One of each party should have
first aid experience.

Duty emergency manager does the following:


• Assembles the emergency organization at the “Emergency Co-ordination Room (ECR)”.
• Organizes additional means (air / ground) if necessary.

Action of Man Lost:


• If your vehicle has been disabled by mechanical failure, an accident or being stuck in sand,
or if you are lost, remember to stay with the vehicle. This will provide shade, protection and
is easier for the search teams to see.
• If possible, park the vehicle on high ground. Clean dust from windows and lights, as this may
help the search teams to see some reflected light from the vehicle.
• Call the camp radio room or rig or Alshawamikh emergency number. If no one answers
repeat your calls thereafter on every half hour. Keep your GMS powered on.
• Keep the outline of your vehicle clearly visible. Shut doors tailboard and bonnet. Mark out
arrows on the ground with stones or items of equipment, which point towards your vehicle.
This will give a larger target to spot from the air.

• Conserve your energy by keeping out of the sun and conserve your water. Leave the tasks
outlined above until the cool of the evening.
• Together with vehicles looking for you there may be, at some stage aircraft involved in the
search. If you see or hear a vehicle or aircraft, make a signal. Even though you cannot see
the source of noise, smoke from a fire can be seen from a considerable distance. Driving
around in circles to create dust will also attract attention. Signaling with a removed driving
mirror is most effective. Do not waste your efforts burning anything if you cannot see or hear
a rescue vehicle or aircraft.
• Do not drink the water from the radiator

Action of Search Teams & Search Coordinator:


• Search crews will log time distance and direction of travel to prevent them also becoming
lost.
• All search vehicles must carry out a mean of communication before leaving camp.
• On the hour and at 30 minutes past each hour at the instructions of the Search Leader all
vehicles will stop and switch off the vehicle lights and engine. Watch for light signals from the
lost party. The Search Leader will interrogate each team following the observation e.g. any
sightings? Any requirements? Any comments?
• If contact is made with the missing persons their location and medical condition should be
ascertained and passed to the Search Leader. If their condition is satisfactory, they should
stay with their vehicle and told that help is on its way. From the information received the
Search Leader will determine, after consultation with the medic, whether to mobilize the
Ambulance.
• At 2400 hours or at the discretion of the Search Leader, and upon his instruction, the search
will be called off for the night. A debriefing is to be held on return of the search parties to
camp.

Action if an Air Search is requested:


• If the lost vehicle is not found using resources of the crew an air search may be organized
through OSC / LEBC. The EC should contact OSC who will request all relevant information
to be passed to LEBC. At this point when outside resources become involved in the search
client will take overall charge of the search and will co-ordinate resources accordingly.
• The crew should prepare a team who should be assembled to travel to the nearest airstrip.
They should be of good eyesight and not liable to airsickness. The more spotters per aircraft
the better. A surveyor with the relevant maps and the proposed search plan is to head each
aircraft search team. A communications vehicle, with a trained flight handler, and the fire
tender will be required to stand by at the airstrip.
• Surveyors may be required to brief the aircraft pilots at the airstrip before takeoff.
• When using aircraft, a search vehicle with no roof markings should be identified by covering
the roof with a blanket or other nominated covering, so that it can be identified from the air
as a search vehicle.

• If the man or men are injured or distressed in any way the ambulance and Medic or First
Aider or HSEA should be dispatched to their location (With support vehicle)
• If the vehicle is broken down or badly stuck, a mechanic will be dispatched to the site to
assist in vehicle recovery.
• If vehicle and personnel are both fit and well, they should return to camp with the search
team which located them.
• Once the man / vehicle is located the search teams, EC, OSC, LEBC & any other third parties
who are involved should be advised that this is the case and that the Emergency is over.

5.1.11 Environmental Spill

Two types of pollution by hydrocarbon spillage should be considered:

Minor pollution: This corresponds to accidental spillage of small hydrocarbon quantities during a
short period.

Major pollution: There is a risk of major pollution each time one of the following conditions occurs:
• Uncontrollable hydrocarbons overflow (pipeline or tank)
• Risk of serious litigation

In case of spill or leak emergency, the emergency controller (EC) shall determine the following:
• Extent of personal injuries, if any,
• Exact location of spill, leak.
• Whether the leak is still occurring and when it was first observed;
• The extent of spill, leak.
• Whether it is necessary to contact ERT.
• Methods to safely control the event.
• If spill containment devices are working.
• If there is any Spill Contaminant Team,

Spill Kit:

Each location should have a spill kit, which should at least include the following:
• 1 x Wheeled Bin
• 6 x Absorbent Socks (3m x 8cm)
• 140 x Absorbent Pads (40cm x 50cm)
• x Absorbent Cushions (30cm x 35cm)

Steps to be taken:
• The emergency alarm has to be raised, all crew members to go to muster point
• EC must confirm with the Driller that the well is secured;
• EC must ensure the Medic or First Aider or HSEA is on location with proper First Aid
equipment;
• EC shall immediately (or delegate someone) contact OSC, also he should inform the
concerned emergency duty manager who in turn will inform the Operations / Project Manager
and HSE department.
• EC to form a Spill Containment team
• The spill containment team to isolate the spill area by roping off as appropriate
• Stop the source of leakage.
• Contain spill if possible, by using spill kit.
• Take appropriate remedial action.

5.1.12 Well Control Situation


Well Control Response Level

Well control events are defined on three response scenario levels, which are described in more
detail below. These are different to Well Control Incident classifications.

• Level 1 Well Control Response

Level 1 response scenarios involve circumstances that are either not uncommon to routine
operations, or do not pose significant risk to personnel and can be resolved, in most instances, using
commonly available equipment, personnel and techniques. Examples of Level 1 scenarios are:

- Well “kicks” (influxes) of manageable volume and intensity (usually < 1.175 kPa/m) within the
limits of the well design / program that are not complicated by mitigating circumstances.

- Mild to moderate loss of circulation.

- Loss of production tubing integrity resulting in sustained pressure on production casing that is less
than 50% of casing burst rating (includes failure of wellhead seals and downhole equipment).

- Loss of production casing integrity resulting in sustained pressure on intermediate casing less
than 25% of casing burst rating (includes failure of wellhead seals).

- Minor surface leaks that can be isolated via remote means or accessed and isolated manually
without significant risk to personnel.

• Level 2 Well Control Response

Level 2 response scenarios involve circumstances that are not commonly encountered during routine
operations, pose the potential for significant risk to personnel, equipment and/or the environment and
require specialized personnel, equipment and/or techniques in order to safely resolve. Examples of
Level 2 scenarios are:

tubing/drill string, washout, plugged choke, etc.

- Lack of fluids or chemicals, e.g. through severe loss of circulation.

- Small surface leaks that cannot be easily or safely isolated.

- Risk of cratering

- Loss of production tubing integrity resulting in sustained pressure on production casing of more
than 50% of casing burst rating (includes failure of wellhead seals and downhole equipment).

- Loss of production casing integrity resulting in sustained pressure on intermediate casing more
than 25% of casing burst rating (includes failure of wellhead seals).

- Loss of protective casing integrity resulting in sustained pressure on surface casing (any
pressure).

- The combined Asset / Well Integrity / Well Engineering RACI in Appendix 8 shall be used for
planning and execution of AC9/10 well integrity events recovery operations.

- CWI Well Control Contingency Plans in Appendix 10&11 (previously in SP1213) shall be used
where required for gas well leak response.

• Level 3 Well Control Response

Level 3 response scenarios are those that present serious risks to personnel, the environment and
assets. These situations require the immediate application of specialized techniques and well-
developed HSE assessment and hazard mitigation programs. Examples of Level 3 scenarios are:

- Surface blowout (drill pipe, BOP, production tree, broach, etc., with or without fire).

- Underground blowout.

- Surface pressure beyond the pressure rating of equipment (including tubulars).

- Other situations that constitute a clear and present danger to personnel, environment or
equipment that cannot be resolved via conventional means.

This WCCP deals with the initial Emergency response and Recovery of a Level 3 Well Control
response scenario.

Level 1 and 2 response scenarios are dealt with under PDO’s SP-1213, Well Control by the
respective UWD departmental teams, with support from PDO’s Well Control Manager and possible
external advice (Group or specialist well control service companies).

• Level 1/2/3 Response Scenarios Communication and Escalation

Procedure to secure the Well & Evacuation:


• Driller & Asst. Driller will stay at Rig floor, wear “S.C.B.A” & perform Shut-In well as per
company defined procedure.
• Emergency controller will immediately inform OSC.
• Driller in-charge / Rig or Site Manager (Emergency Controller) will confirm that well shut-in
successfully.
• After securing the well Driller & Assistant Driller will proceed to “Assembly Point”.
• Emergency response team will equip them self & get ready for any unexpected firefighting.
• Rig medic or First Aiders will report at assembly point along with emergency trauma bag.
• Ambulance will be brought near “Assembly point” by on duty driver.
• OSC will notify LEBC for help, if required by Driller
• Muster controller will conduct headcount at assembly point.
• In case of missing crew, emergency response teams will start searching for missing
personnel.
• EC & OSC will look after all operation directly.

5.1.13 Major Injury or Illness

• The EC must ensure the Medic or First Aider or HSEA is on location with proper First Aid
equipment;
• The OSC to immediately contact LEBC
• EC shall inform the concerned Operations / Project Manager who in turn will inform CEO
• If EC & OSC via medic have to decide about evacuation.
• If evacuation required, medic shall accompany the causalities in ambulance
• Take all the causalities to nearest Client clinic.

5.1.14 Fatality
• The emergency alarm has to be raised, all crew to go to muster point
• The Emergency controller must confirm with the Driller that the well is secured
• EC shall immediately inform OSC.
• The EC must ensure the Medic or First Aider or HSEA is on location
• The EC to immediately contact local police
• The OSC should contact LEBC
• The EC should inform the concerned Field Superintendent or Operations / Project Manager
who in turn will inform CEO / QHSE Manager
• The EC & OSC have to arrange for transportation of the deceased person to the nearest
client medical facility.

5.1.15 Rig Failure


Rig failures include:

• Sub structure collapse


• Mast failure
• Rig foundation collapse
• Draw works breakdown
• Complete mud pump failure
• Total power failure

After a rig failure occurred, the courses of action will be as follows:

shall be called to the incident scene.


• OPERATOR drilling supervisor or company man shall be informed and all assistance
shall be provided to him for further processing in the emergency situation.
• Once a rig failure has taken place, the TP, in coordination with the company man
(Operator drilling supervisor), must establish if the well can be shut in by considering
the following:
- Are the BOPs functioning normally?
- Is the BOP accumulator still operating?
- Can a stab-in valve or Kelly cock be installed in the string?
• In case answer to any of the following is negative, first considerations must be given
to restoring the capability of securing the well.
• If secondary well control is not possible, then the first priority must be ensuring the
well can be secured. This can either be achieved by restoring secondary well control
or by plugging the well bore using a cement or barite plug.
• If the string in the well cannot be moved any cement job will result in cement left in
the pipe and will require remedial operations at a later date and must therefore only
be carried out as a last resort.
• In all cases the information communication channel as indicated in the flow chart
must be followed.

5.1.15 Radiation
Chemical radioactive sources are used in wireline electric logging tools. Radiation can also occur naturally
and is known as NORM (Naturally Occurring Radio-active Material). For more details refer to PR-IMS-014
NORM.

A radiation emergency has occurred if the following is present:


• Radiation Overexposure
• Radiation Released (Accessible High Dose Rates)
• Radioactive Source Lost, Stolen or Mislaid
• Contaminated Area (Tracer Material)
• Fire in Area where Radioactive material is used or stored
• Radioactive material during rig emergency.

Competent Person
The Competent Person is one who is trained and certified to work with the Radioactive Sources used by his
company. He is also aware of Local Regulations concerning use of Sources. As the majority of radioactive
sources used on the rig belong to the electric logging and MWD/LWD contractors, the competent person is
therefore the logging engineer. He is responsible for transportation, storage and handling of the sources. In
the event of a radiation emergency, he is required to implement his own company Emergency Procedures.
He must provide the OSC with all relevant information on the source.

5.1.16 Food Poisoning


Food Incident

The circumstances surrounding the identification of a hazard which poses a risk to health

and can be considered as (but not exclusively):

•An event which had potentially harmful implications to the public resulting from the consumption of food.

• The identification of contaminated food, that if consumed may lead to illness.

• The identification of serious human illness that may be linked to contaminated food.

• The identification of unhygienic practices by the caterer of such a degree as to present an immediate danger
to consumers’ health.

Where the scale and complexity of an incident is such that some degree of other official entity co-ordination
and support is necessary, Location ERT will be responsible for the overall management of the food incident
and involvement of official entity co-ordination.

The ERT will then accordingly:


• Gather and evaluate available information
• Identify hazards and assess risk
• Initiate action to the response plan
• Initiate risk assessment &/or investigation
• Decide risk management activities
• Identify stakeholder groups to be involved
• Request further information as necessary
• Prepare communication to stakeholders and employees

5.1.17 Explosions
This section addresses explosion(s) that has taken place due to the result of an unnoticed gas leak. The
aftermath of an explosion is fire. The first priority in responding to an explosion is the safeguarding of
personnel, evacuating those who are injured and commencing Search and Rescue IF SAFE TO DO SO

5.1.18 Chemical Spills

Chemical spillage (Acidic, oil or drilling fluids) may have detrimental impact on life, the environment or
assets and may require area or national resources of specialized third parties.

The chemicals discharge will normally be a vapour or a liquid that gives off fumes of the acidic gas. Reaction
of HF and HCl with certain metals generates flammable and potentially explosive hydrogen gas. In addition,
considerable heat is evolved when contacted with many substances. The aforementioned heat may increase
the pressure inside the container and violent rupture of the container can result.

5.1.20 Means of Recovery to a Place of Safety

Suitable arrangements have been established to get people evacuating from operating sites to a
place of safety.

The primary ‘place of safety’ is the campsite, to which personnel can be transported using
company-provided road vehicles, i.e. buses and cars.

Medical evacuation to the nearest hospital is available in accordance with the Alshawamikh Medical
Evacuation (Medevac) Plan.
