CRTICICAL INCIDENT ANALYSIS

On 12 January 2023 in CCU . I saw an incident happened between staff nurse and
patient attendent. Staff nurse passed I/V canola to the patient wrong side. She
pricked second time in veins but cannot adjust very well. So this situation saw the
patient attendent and he hyper on staff nurse and both fight to each other. He
complained the duty doctor and he angry with staff nurse and said next time
carefully work.
ANALYSIS
Patient attendent behave like a staff is very careless and not responsible. The
doctor was intentionally degrading nurses.
EVALUATION
The incident was noticeable because the nurses were breaking the rules and not
improve her skills. Also the behavior of patient attendent was not fair.
CONCLUSION
A. General conclusion
I learnt from the staff Nurse skills was not good and this condition is not good for
patient because it’s pain full procedure and also lose the blood of patient. This
situation the first priority of patients health.
A) Action plan
If I was in that situation I talk with patients family very politely and excuse my
work and do my best work in next time.
RECOMMENDATION
I recommended that before misbehaving you must have to clarify the issue bring
involved parties together and identify a solution.
LITRETURE REVIEW
Peripheral intravenous catheters (PIVCs) are the most widely used invasive device
in hospitals worldwide [1]. These devices can lead patients to experience multiple
complications during the insertion, maintenance and management of intravenous
therapy during a hospital admission [1–3]. PIVCs are indicated for short-term use,
usually around a week. However, up to 69% of PIVCs are prematurely removed
due to PIVC failure, defined as unplanned PIVC removal with mechanical
complications (phlebitis, occlusion, infiltration) or infection before the completion
of any scheduled intravenous therapy
