MEDICAL ABSTRACT
MARCH 1, 2023

General Data This is a case of CONTILLO, MARK YIAN BABATIO, 16 years old, Male, Single,
Roman Catholic, Filipino, from Brgy. 96 Calanipawan, Tacloban City, Leyte, was
admitted at our center last February 20, 2023 at 8:38 PM.

Chief Complaint Abdominal Pain

History 3 weeks prior to admission, patient had onset of Chicken pox and fever Tmax 38⁰C
resolved by Paracetamol 500mg/tab and rest. No consult done.
2 weeks prior to admission, patient experienced knee pain, bilaterally, bipedal
edema grade 2+ on ankle area, red skin lesions on both legs and body malaise. No
medications taken. In the interim, persistence of symptoms was noted and patient
has difficulty in doing activities of daily living without support. Still no medications.
Consult done at Tacloban City Hospital. Diagnostics were requested and results
were unremarkable.
3 days after the said consult, onset of generalized abdominal pain with a pain rate
scale of 10/10 associated with vomiting postprandial with blood 3x, fever,
headache and 3 episodes of loose watery bowel movements.
1 week prior to admission, worsening of abdominal pain still with a pain rating
scale of 10/10. Unable to sit, stand and walk due to weakness and pain. This
prompted admission at Tacloban City Hospital. Upon admission, diagnostics were
requested. Ultrasound revealed bilateral renal parenchymal disease and ASO
576.94 H. He was given Ampicillin and Sulbactam, Furosemide, Captopril, and
Hyoscine. No improvement of patient prompted referral to RTRH for further
management.

Medications 1. Co-Amoxiclav 625mg/tab, 1 tab BID x 3 days


2. Furosemide 40mg IVTT q6h
3. Cefotaxime 1g IVTT q8h
4. Sodium Bicarbonate 50meqs + equal diluent D5W q8h
5. Multivitamins + ADE 1 cap OD
6. UDCA 500mg/tab
7. Spironolactone 100mg/tab
8. Nifedipine 30mg/cap 1 cap q8h
9. Sodium Bicarbonate 650mg/tab BID
10. Tranexamic Acid 500mg/cap 1 cap q8h
11. Amikacin 250mg IVTT q12h
12. Metolazone 5mg/tab 1 tab BID x 5 days

Working Diagnosis: Acute Kidney Injury; Renal Parenchymal Disease I; Post-Streptococcal Acute
Glumerulonephritis; T/C Leptospirosis; Late Adolescent; No wasting, No
stunting
