SUB – CONTRACTORS AFFIDAVIT OF NO LIABILITY

I, (SUB-CONTRACTOR), of legal age, Filipino, (STATUS), and a resident of (ADDRESS), after having been duly
sworn in accordance with law, do hereby depose and state that:

1. That I am the Contractor of DRAZEN TAROMA CONSTRUCTION SERVICES, a company duly registered
and existing in accordance with the laws of the Republic of the Philippines.

2. The Contractor has contracted with DRAZEN TAROMA CONSTRUCTION SERVICES for the construction
for its project ST. HONORE, Iloilo Business Park, Megaworld Blvd., Mandurriao, Iloilo City

3. The Contractor has paid all wages of its employees. All bills for labor, and has complied with the existing labor
laws and standards and all other laws appurtenant thereto.

4. The Contractor has also paid all insurances, expenses and all taxes due to the government by reason of the
foregoing project.

5. That no government equipment was used or being used in the project.

6. The Contractor had no given or promised any consideration in kind or cash to anybody who has something to do
with the above-mentioned contract.

7. The Contractor shall hold the DRAZEN TAROMA CONSTRCUTION SERVICES free and harmless from: (a)
any and all claims of, or liabilities to, Contractor’s employees or workers, agents, representatives for shares or
salaries and wages, employees’ benefits, compensation for death, injury, or sickness, or for any other claim arising
from or in connection with their employment by Contractor; and (b) claims made by the Contractor’s suppliers for
unpaid materials, supplies, equipment and tools. The Contractor assumes all liability for such claims and shall
indemnify the Company for all expenses it may incur defending itself therefrom.

8. That I am executing this Affidavit of Warranty and No Liability for the purpose of collection for the whole contract
in state of progress billing.
