We hereby certify that the equipment described behalf of DCD (certificate No. T-16/03/2021-HZ-254) in this certificate was tested
& thoroughly examined by a competent person in accordance with applicable BS Standards & Manufacturer recommendation. At the
time of inspection, the Equipment has been found to be free from any adverse qualities that can affect its safe use and it is safe for its
intended use. DASCO or its inspector will not be liable to any changes in any circumstances.
Notes:
• The validity of the certificate is 12 months, in the event of any repair or modification or alteration made, in which case it must be Re-inspect as per local
requirements.
• The inspection certificate shall not be reproduced except in full without the approval of the inspection body DASCO.
• After every inspection, a record should be maintained by the operator and kept by the user/owner.
